摘要
目的:探讨胰腺实性假乳头状瘤(solid pseudopapillary tumor of the pancreas,SPTP)的CT及MRI特征性影像表现,提高该病的影像诊断符合率。方法:回顾性分析海安市人民医院及复旦大学附属中山医院2014年5月至2017年5月期间23例经病理证实的胰腺实性假乳头状瘤患者的影像资料,13例同时行CT和MRI检查,其余10例中5例仅行CT检查,另5例仅行MRI检查。结果:所有病例均为单发,境界清,胰尾11例,胰头6例,胰颈4例,胰体2例;囊实性18例,实性4例,囊性1例;其中12例有出血,9例有钙化;增强扫描肿瘤均为乏血供,增强后实性区呈轻至中度延迟强化,囊性区无强化,均无主胰管扩张。结论:SPTP的CT及MRI影像表现具有一定特征性,对术前临床定性诊断有很大的价值。
Objective:To explore CT and MRI manifestations of solid pseudopapillary tumor of the pancreas(SPTP),and to improve diagnostic accuracy.Methods:Twenty-three pathologically-confirmed cases of SPTP in the People's Hospital of Haian and Zhongshan Hospital of Fudan University were retrospectively analyzed,thirteen cases were given CT and MRI scanning.And five of the other ten cases were examined merely with CT as well as five of the other ten cases only under MRI examination.Results:The origin of the tumor in all patients were single and regular margins.Eleven were located in the pancreatic tail,six in head,four in neck and two in body.Eighteen were solid and cystic tumors,four were solid tumors,one was cystic tumors.Well-defined hemorrhage in twelve cases,and calcification were found in nine cases.All tumors were hypovascular,contrast enhanced scanning showed the solid component were enhanced which showed mild to moderate enhancement in delayed phase and cystic component were no enhancement.No secondary duct dilatations were detected.Conclusion:There are certain characteristics of the the imaging features of SPTP.There would be a great help to the diagnosis preoperatively.
