摘要
目的探究应用关节灌注冲洗术联合玻璃酸钠注射对膝骨关节炎患者膝关节功能、视觉模拟评分(VAS)和生活质量的影响。方法选取2015年3月至2018年3月就诊于四川省骨科医院的膝骨关节炎患者68例为研究对象,按照随机数字表法分为对照组和病例组,每组34例。对照组患者予以玻璃酸钠注射治疗,病例组患者予以关节灌注冲洗术联合玻璃酸钠注射治疗,两组患者均每周治疗1次,连续治疗5周。比较两组患者治疗前、治疗5周后膝关节功能[国际膝关节文献编制委员会膝关节评估表(IKDC评估表)]、疼痛感觉(VAS)、生活质量[健康状况调查简表(SF-36)]及炎症因子[白细胞介素-1β(IL-1β)、白细胞介素-17(IL-17)、转化生长因子-β(TGF-β)]水平差异。结果治疗5周后,两组患者IKDC、SF-36量表评分、TGF-β水平均较治疗前上升;VAS评分、IL-1β、IL-17水平均较治疗前下降,差异均有统计学意义(P<0.05);且病例组上述指标变化均大于对照组(P<0.05)。结论关节灌注冲洗术联合玻璃酸钠注射能有效改善膝骨关节炎患者膝关节功能、疼痛感及生活质量,并上调抗炎性因子,下调促炎性因子,较单独注射玻璃酸钠治疗效果明显。
Objective To investigate the effect of joint perfusion irrigation combined with sodium hyaluronate injection on the knee joint function,visual analogue scale(VAS)and quality of life in patients with knee osteoarthritis.Methods A total of 68 cases with knee osteoarthritis treated in Sichuan Provincial Orthopaedic Hospital from March of 2015 to March of 2018 were selected as the subjects and divided into the control group with 34 cases in each group and case group according to the random number table method.The control group was treated with sodium hyaluronate injection while the case group was treated with joint perfusion irrigation combined with sodium hyaluronate injection.Both groups were treated once a week for 5 weeks consecutively.The knee function by International Knee Documentation Committee Knee Evaluation Form(IKDC Evaluation Form),pain feeling by Visual Analogue Scale(VAS),quality of life by Short-form Health Status Survey Scale(SF-36)and levels of the inflammatory factors including interleukin-1β(IL-1β),interleukin-17(IL-17)and transforming growth factor-β(TGF-β)before treatment and after 5 weeks of treatment were compared between the two groups.Results After 5 weeks of treatment,the scores of IKDC and SF-36 and the level of TGF-β were significantly increased in the two groups when compared with those before treatment(P<0.05)while the VAS score and the levels of IL-1β and IL-17 were significantly decreased when compared with those before treatment(P<0.05),and the changes of all those indicators in the case group were significantly greater than those in the control group(P<0.05).Conclusion Joint perfusion irrigation combined with sodium hyaluronate injection can effectively improve the knee joint function,pain feeling and quality of life in patients with knee osteoarthritis by up-regulating the anti-inflammatory factors and down-regulating the pro-inflammatory factors,and its therapeutic effect is more obvious than that of sodium hyaluronate injection.
