摘要
目的探讨高血压患者并发症的发生情况与相关特征,为制定更为精准的高血压综合防控策略提供参考依据。方法收集上海市金山工业区2007～2017年进行建卡管理的高血压患者资料,分析11年来已管理的高血压患者不同性别、不同年龄组间并发症发生的种类与发生率分布特征。结果高血压患者主要是以60岁以上老年人为主,占77. 40%。高血压患者并发症的发生率为38. 28%;并发症种类中45. 73%为糖尿病,36. 99%为脑血管损害,26. 34%为心脏损害;脑血管损害主要是缺血性脑卒中和短暂性脑缺血发作;心脏疾病主要是心绞痛和心肌梗死。不同性别高血压患者并发症发生率差异无统计学意义(χ~2=3. 20,P=0. 08),不同年龄组间高血压患者并发症发生率差异有统计学意义(χ~2=143. 99,P=0. 00)。在出现并发症的高血压患者中,17. 7%的患者出现2种并发症,4. 37%的患者出现3种及以上并发症。结论高血压患者合并心脑血管病的发生率较高,在高血压管理中应采取综合防控措施,最大限度地降低高血压患者心脑血管病等并发症的发生率。
Objective To investigate the incidence and characteristics of complications in hypertensive patients,and provide reference for developing more accurate community hypertension prevention and control measures.Methods Collected data of hypertensive patients from 2007 to 2017,and The types and incidence distribution of complications were analyzed.Results The majority of hypertension patients were over 60 years old,accounting for 77.40%.The incidence of complications in hypertensive patients was 38.28%,Of which 45.73%were diabetes,36.99%for cerebrovascular damage,26.34%for heart damage and cerebral vascular injury mainly includes ischemic stroke and TIA,and heart disease mainly includes angina pectoris and myocardial infarction..There was no significant difference in the incidence of complications among hypertensive patients of different gender(χ^2=3.20,P=0.08).The incidence of complications among hypertensive patients in different age groups was statistically significant(χ^2=143.99,P=0.00).Among the patients with hypertension complications,17.7%of patients had 2 complications,4.37%of them had 3 or more complications.Conclusion There is a high incidence of cardiovascular and cerebrovascular diseases in hypertension patients,comprehensive prevention and control measures should be strengthened to reduce the incidence of cardiovascular and cerebrovascular diseases.
