摘要
目的探讨PDCA管理模式对PICC置管患者并发症预防及自我管理的影响。方法回顾性分析81例PICC置管患者的病历资料,观察组41例患者采用PDCA管理模式,对照组40例患者采用常规管理模式。观察比较两组患者住院时间、导管留置时间、并发症发生情况、患者满意度以及管理前后自我管理评分情况。结果对照组患者并发症发生率为42.50%(17/40),高于观察组的17.07%(7/41),差异有统计学意义(P﹤0.05)。观察组患者住院时间明显短于对照组患者,差异有统计学意义(P﹤0.01);观察组患者导管留置情况明显优于对照组,差异有统计学意义(P﹤0.01)。管理前,两组患者自我管理评分比较,差异无统计学意义(P﹥0.05);管理后,对照组患者自我管理评分明显低于观察组患者,差异有统计学意义(P﹤0.01)。观察组患者管理后满意度为95.12%(39/41),高于对照组的77.50%(31/40),差异有统计学意义(P﹤0.05)。结论实施PDCA管理模式可降低PICC置管患者并发症发生率,缩短住院时间,提升患者自我管理水平,提高患者满意度,对改善医患关系具有积极的促进意义,值得临床推广应用。
Objective To explore the effect of PDCA management model on the prevention of complications of PICC and patients’self-management. Method A retrospective analysis containing 81 cases with PICC was performed, 41 cases in the study group were treated based on PDCA management model, while the 40 cases in control group received routine management. The length of hospital stay, duration of indwelling catheterization, occurrence of complications, satisfaction rate and self-management score before and after interventions were compared. Result The incidence of complications in the control group was 42.50%(17/40), which was significantly higher than that in the study group at 17.07%(7/41), with statistically significant difference observed (P<0.05). The length of hospital stay was significantly shorter in study group than that in control group (P<0.01);while longer duration of indwelling catheterization was observed in study group compared with control group, the difference was of statistical significance (P<0.01). Before intervention, the selfmanagement scores in both groups were similar (P>0.05), while it was significantly lower in control group compared with that of study group after interventions (P<0.01). The satisfaction rate in study group was 95.12%(39/41) after intervention, and was significantly higher than that of the control group at 77.50%(31/40), showing statistically significant difference (P<0.05). Conclusion The application of PDCA management model can reduce the incidence of complications in patients administered with PICC, shortening the time required for hospital stay, improving the level of self-management, with higher satisfaction rate of patients, thus it is of positive significance to improve the doctor-patient relationship and is worthy of clinical application.
