摘要
目的探讨分析老年胆囊结石伴胆囊炎开腹胆囊切除术与腹腔镜胆囊切除术的临床疗效。方法选取2014年2月—2017年2月在我院接受治疗的老年胆囊结石伴胆囊炎患者60例分为观察组(腹腔镜胆囊切除术)和对照组(开腹胆囊切除术)各30例。对比两组患者手术效果、手术相关指标以及相关并发症的发生情况。结果观察组临床总有效率达到96.67%,对照组患者临床总有效率为70.00%,观察组临床总有效率高于对照组,差异具有统计学意义(P <0.05);观察组患者手术切口长度短于对照组,手术时间和术中出血量以少于对照组,差异具有统计学意义(P <0.05);观察组患者术后排气时间、下床活动时间、住院时间均少于对照组,差异具有统计学意义(P <0.05);观察组患者并发症发生率为13.33%,对照组患者并发症发生率为36.67%,差异具有统计学意义(P <0.05)。结论老年胆囊结石伴胆囊炎患者实施腹腔镜胆囊切除术,可取得良好临床效果。
Objective To investigate the clinical efficacy of open cholecystectomy and laparoscopic cholecystectomy in elderly patients with cholecystolithiasis and cholecystitis.Methods 60 patients with cholecystolithiasis and cholecystitis who were treated in our hospital from February 2014 to February 2017 were enrolled in the observation group(laparoscopic cholecystectomy)and the control group(open cholecystectomy).The surgical outcomes,surgical related indicators,and related complications were compared between the two groups.Results The total effective rate of the observation group(96.67%)was significantly higher than that of the control group(70.00%),the difference was statistically significant(P<0.05).The incision length of the observational group was shorter than that of the control group,the operation time and bleeding amount during operation in the observation group was fewer than those in the control group,the difference was statistically significant(P<0.05).The time of exhaust after operation,out of bed and hospitalization of the patients in the group was shorter than those in the control group(P<0.05).The incidence of complications in the observation group(13.33%)was significantly lower than that in the control group(36.67%)(P<0.05).Conclusion Laparoscopic cholecystectomy in elderly patients with cholecystolithiasis and cholecystitis can achieve good clinical results.
