摘要
目的探讨青光眼合并白内障同期行联合手术的临床疗效。方法方便选取该院在2013年1月—2017年12月期间收治的84例青光眼合并白内障患者,将其随机分为研究组和对照组,各42例,对照组先进行青光眼滤过术,1个月后再行白内障超声乳化术,研究组同期进行青光眼滤过术联合白内障超声乳化术,比较两组术前术后视力、眼压变化及并发症情况等。结果研究组术后1周视力(0.5±0.1)、术后1个月(0.5±0.1)、术后3个月(0.7±0.3)、术后半年(0.7±0.3)与对照组比较差异有统计学意义(t=4.133、4.133、4.522、4.062,P<0.05);研究组术后眼压(15.2±2.3)mmHg与对照组比较差异有统计学意义(t=4.362,P<0.05),与组内术前比较差异有统计学意义(t=5.899,5.276,P<0.05);研究组术后视觉质量自我评价满意率90.48%显著高于对照组71.43%(χ~2=6.893,P<0.05);均无严重并发症发生。结论对青光眼合并白内障同期行联合手术,效果显著,促进患者视力恢复,维持良好的眼压,安全性好,患者术后视觉质量自我评价满意率,值得临床进一步推广。
Objective To study the clinical curative effect of simultaneous combined surgery in treatment of glaucoma and cataract.Methods 84 cases of patients with glaucoma and cataract admitted and treated in our hospital from January 2013 to December 2017 were convenient selected and randomly divided into two groups with 42 cases in each,the control group underwent the glaucoma filtering surgery firstly and phacoemulsification after 1 month,while the research group underwent the glaucoma filtering surgery combined with phacoemulsification,and the vision,eye pressure changes and complications before and after surgery were compared between the two groups.Results The visions in 1 week,1 month,3 months and 6 months after surgery in the research group were respectively(0.5±0.1),(0.7±0.1),(0.7±0.3),(0.7±0.3),and the differences were statistically significant(t=4.133,4.133,4.522,4.062,P﹤0.05);and the postoperative eye pressure in the research group was(15.2±2.3)mmHg,and the difference between the two groups was statistically significant(t=4.362,P﹤0.05),and the difference between groups before surgery was statistically significant(t=5.899,5.276,P﹤0.05),and the self-evaluation satisfied rate of vision quality after surgery in the research group was obviously higher than that in the control group 90.48%vs 71.43%(χ^2=6.893,P﹤0.05),without serious complications.Conclusion The effect of simultaneous combined surgery in treatment of glaucoma and cataract is obvious,which can promote the vision recovery,and maintain a good eye pressure,and the safety is good,and it can also improve the postoperative vision quality self-evaluation satisfactory rate,and it is worth further promotion in clinic.
