摘要
目的 探讨泪道激光联合植管术对泪道阻塞患者眼表及神经肽P物质(SP)的影响。方法 选取2017年9月至2018年9月期间本院收治的86例患者,按照随机数表法分成对照组和研究组,每组43例(75眼)。其中对照组患者采取单纯泪道激光术,研究组患者在此基础上联合给予植管术进行治疗。对两组患者进行随访,观察术后患者的临床治疗效果,记录患者术前及术后泪膜破裂时间、泪液分泌量和泪液中神经P物质的含量。结果 研究组总有效率显著高于对照组(93.33%比81.33%),两组比较差异有统计学意义(P<0.05);对照组和研究组患者治疗前后的泪膜破裂时间和泪液分泌量比较差异均无统计学意义(均P >0.05);对照组患者治疗前泪液中神经肽P物质含量为(258.43±17.35)pg/ml,治疗后为(262.43±14.74)pg/ml,两者比较差异无统计学意义(P >0.05);而研究组患者治疗后泪液中神经肽P物质从(253.38±16.04)pg/ml增加到(305.64±27.53)pg/ml,两者比较差异有统计学意义(P<0.05)。结论 泪道激光联合植管术对泪道阻塞患者有良好的治疗效果,对患者的眼表无显著影响,但可导致患者泪液中SP质含量升高,可能与该物质在术后参与创伤组织修复过程有关。
Objective To observe the clinical effect of laser canalicular plasty combined with drainage tube implantation on ocular surface and neuropeptide P in patients with lacrimal duct obstruction.Methods 86 patients with lacrimal duct obstruction treated in our hospital from September 2017 to September 2018 were randomly divided into 2 groups,43 patients(75 eyes)in each group.The patients of the control group were treated with laser canalicular plasty,while those of the treatment group were given drainage tube implantation additionally based on the control group.All cases were followed up.After treatment,the clinical effect of both groups were observed.In addition,ocular surface was assessed,which included tear film rupture time,tear secretion,and the content of neuropeptide P.Results After treatment,the clinical curative effect of the treatment group(93.33%)was better than that of the control group(81.33%)(P<0.05).However,there were no statistically significant differences in tear film rupture time and tear secretion between the control group and the treatment group before and after treatment(P >0.05).The content of neuropeptide substance P of the control group did not change significantly after treatment[(258.43±17.35)pg/ml before treatment,(262.43±14.74)pg/ml after treatment](P >0.05);while the content of neuropeptide substance P of the treatment group after treatment[(305.64±27.53)pg/ml]was significantly higher than that before treatment[(253.38±16.04)pg/ml](P<0.05).Conclusions Laser canalicular plasty combined with drainage tube implantation can improve the clinical effect in the treatment of lacrimal duct obstruction,which has no significant effect on ocular surface,but can increase the content of the neuropeptide P,which may due to the SP plays a role in the issue repair process after surgery.
