摘要
目的探讨无创机械通气在老年肺动脉高压患者呼吸衰竭中的应用效果。方法选取100例老年肺动脉高压并发呼吸衰竭患者分为两组,观察组(n=50)给予无创机械通气,对照组(n=50)给予有创机械通气,对比两组患者治疗效果。结果治疗后两组患者内皮素-1、白细胞介素-6、C-反应蛋白水平均明显下降,但观察组患者下降更为明显(P <0.05);治疗后观察组患者脑钠肽水平明显下降(P<0.05)。治疗后两组患者氧合指数、动脉血二氧化碳分压均明显下降,动脉血氧饱和度、动脉血氧分压明显提高,且观察组患者变化更为明显(P <0.05)。两组患者并发症发生率16.00%/34.00%对比差异明显(P <0.05)。结论对肺动脉高压并发呼吸衰竭患者进行无创机械通气,能有效避免医源性损伤,改善患者氧合功能。
Objective To explore the effect of noninvasive mechanical ventilation in the treatment of respiratory failure in elderly patients with pulmonary hypertension.Methods 100 elderly patients with pulmonary hypertension complicated with respiratory failure were divided into two groups.The observation group(n=50)was given non-invasive mechanical ventilation,and the control group(n=50)was given invasive mechanical ventilation.Results The levels of endothelin-1,interleukin-6 and C-reactive protein in the two groups were significantly decreased after treatment,there was significant difference between two groups(P<0.05),the level of brain natriuretic peptide in the observation group was significantly lower than that in the control group(P<0.05).After treatment,the oxygenation index and arterial partial pressure of carbon dioxide in both groups were significantly decreased,arterial oxygen saturation and arterial partial pressure were significantly increased,and the changes in the observation group were more obvious(P<0.05).The incidence of complications in the two groups was significantly different from 16.00%/34.00%(P<0.05).Conclusion Noninvasive mechanical ventilation can effectively avoid iatrogenic injury and improve oxygenation function in patients with pulmonary hypertension complicated with respiratory failure.
