摘要
目的:探讨肝细胞肝癌(HCC)术前多层螺旋CT(MSCT)征象与其术后复发的相关性,初步评估MSCT对HCC术后复发的预测价值。方法:回顾性分析2013年7月至2015年7月在广西医科大学附属肿瘤医院行根治性切除术的73例HCC患者术前MSCT资料,其中2年内复发24例(复发组),未复发49例(未复发组)。比较两组MSCT征象(瘤灶大小、位置、瘤周强化征、瘤灶形态、肿瘤包膜类型)差异,采用Logistic回归分析法分析复发组术前各种MSCT征象与术后复发的相关性。绘制受试者工作特征(ROC)曲线,分析术前MSCT对HCC复发的预测效能。结果:两组瘤灶大小、位置比较差异无统计学意义(P>0.05),瘤周强化征、瘤灶形态、肿瘤包膜类型比较,差异有统计学意义(P<0.05)。瘤灶形态和肿瘤包膜类型是HCC术后复发的独立危险因素(P<0.05)。瘤灶形态、肿瘤包膜类型及两种征象联合预测HCC术后复发的灵敏度、特异度、准确度分别为60.87%、86.00%、70.93%,80.96%、60.00%、80.28%及86.96%、78.00%、94.37%。结论:HCC术前MSCT瘤灶形态和肿瘤包膜类型对HCC术后复发具有一定的预测价值,且两种征象联合预测的准确度更高。
Objective:To investigate the correlation between pre-operative multislice computed tomography(MSCT)signs and postoperative recurrence of hepatocellular carcinoma(HCC).Methods:The pre-operative MSCT examination results of 73 HCC patients including 24 cases with recurrence(recurrence group)and 49 cases without recurrence(non-recurrence group)were retrospectively analyzed.MSCT signs(tumor size,location,peritumoral enhancement sign,tumor morphology,tumor capsule type)were compared between the two groups.The correlation between pre-operative MSCT signs and HCC recurrence was analyzed by logistic regression analysis.The value of MSCT signs for predicting recurrence was evaluated using the receiver operating characteristic(ROC)curve.Results:There were no significant differences in tumor size and tumor location between the two groups(P>0.05),and there were significant differences in peritumoral enhancement sign,tumor morphology and tumor capsule type between the two groups(P<0.05).Tumor morphology and capsule type were independent risk factors for postoperative HCC recurrence.The sensitivity,specificity and accuracy of tumor morphology,tumor capsule type and the combination of two signs in predicting HCC recurrence were 60.87%,86.00%,70.93%;80.96%,60.00%,80.28%and 86.96%,78.00%,94.37%,respectively.Conclusion:MSCT signs such as tumor morphology and tumor capsule type were valuable for predicting HCC recurrence,and the two signs combination had higher predictive accuracy.
