摘要
目的分析多学科协作联合腹腔镜手术治疗老年直肠癌的特点及效果。方法选取100例老年直肠癌患者,随机分为观察组与对照组。对照组采用新辅助化疗联合腹腔镜手术治疗,观察组在对照组的基础上加入辅助化疗,比较两组患者的临床疗效、术后并发症发生率及术后拔除胃管、尿管、引流管的时间,并采用问卷调查分析患者对治疗的满意度。结果观察组与对照组的临床疗效显著优于对照组(P <0.01);观察组并发症发生率为10.0%,对照组并发症发生率为22.0%,两组间比较差异有统计学意义(P <0.01);观察组术后拔除胃管、尿管、引流管的时间均显著短于对照组(P <0.01),且观察组患者的满意率也显著优于对照组(P <0.01)。结论多学科协作模式下,新辅助化疗、辅助化疗联合腹腔镜手术在老年直肠癌患者中具有较高的应用价值,能够缩短提高临床疗效以及患者的满意度,改善老年患者的生活质量。
Objective To analyze the characteristics and effects of multidisciplinary collaboration combined with laparoscopic surgery in the treatment of rectal cancer. Methods From January 2012 to December 2016,100 elderly patients with rectal cancer were randomly divided into observation group and control group.The patients in the control group were treated with neoadjuvant chemotherapy combined with laparoscopic surgery.The observation group was treated with adjuvant chemotherapy on the basis of the control group.The clinical curative effects,postoperative complication rates and postoperative removal of gastric tube,ureter,drainage tube,patient satisfactions with treatment by questionnaires were analyzed. Results The clinical curative effect of the observation group and the control group was significantly better than that of the control group ( P <0.01).The complication rate was 10.0% in the observation group and 22.0% in the control group.The difference between the two groups was statistically significant ( P <0.01).The rate of removal of gastric tube,ureter and drainage tube in the observation group was significantly shorter than that in the control group ( P <0.01),and the satisfaction rate of the observation group was also significantly better than that of the control group ( P < 0.01). Conclusion Neoadjuvant chemotherapy,adjuvant chemotherapy combined with laparoscopic surgery has a high value in elderly patients with rectal cancer.It can shorten the improvement of clinical curative effect and patient satisfaction and improve the quality of life of elderly patients.
