运动锻炼干预对我国中老年女性骨密度影响的meta分析 被引量:13

摘要 系统评价运动锻炼干预对我国中老年女性骨密度的影响,以期提供最优的健骨运动处方。方法:计算机检索中国学术期刊网(CNKI)、万方(WAN-Fang)数据库、中文科技期刊全文数据库(VIP)等,检索期限均从各数据库建库开始至2017年1月15日,收集运动锻炼影响中老年女性骨密度的随机对照实验(RCT),应用Cochrane偏倚风险评估工具,采用Revman5.3软件对纳入的研究进行方法学质量评价,采用stata12.0软件进行统计分析。结果:共有15篇RCT文献被纳入,实验被试包括703名中老年女性。meta分析结果显示,与对照组相比,规律性参加体力活动其骨密度明显提高(SMD=0.57,95%CI:0.32~0.83),且具有统计学意义,但其异质性较大I2=60%。通过meta回归分析和亚组分析进一步探索其异质性来源,结果发现实验对象年龄不同是异质性的来源;亚组分析中研究文献质量高,其同质性好。为了确定研究结果的可靠性及稳健性,又对其进行敏感性分析,结果显示:逐个剔除单项研究后,效应量仍在95%CI以内,加强了原meta分析结果的可靠性。此外,追踪最优效应量的原研究文献得出:最优健骨运动处方为健步走,运动锻炼强度为60%~80%HRmax,干预周期为15周,每周3次,每次60 min左右。结论:运动锻炼能有效改善我国中老年女性骨密度,延缓骨质流失。建议我国中老年女性按照所提供的健骨运动处方进行锻炼。 Objective: To evaluate the effect of exercise intervention on bone mineral density of middle-aged and old women, in order to provide the most optimal bone-invigorating exercise prescription. Methods: Using computer to retrieve CNKI, Wan-Fang, VIP database, with the retrieval time from the start of database building to January 2017, so as to collect exercise intervention on bone mineral density in middle-aged and old women randomized controlled trials. Using the Cochrane bias risk assessment tools to evaluate the methodological quality of included studies, Revman5.3 software for methodological quality assessment included in the study and having a statistical analysis with stata12.0 software. Result: There are 15 research literatures, 703 middle-aged and old women included. The results of meta-analysis showed that compared with the control group, the bone mineral density with regular physical activity was significantly increased(SMD=0.57,95%CI:0.32 ~0.83),and it had statistical significant, but its heterogeneity was greater I2= 60%. Through meta regression analysis and subgroup analysis to further explore its heterogeneity source, the results found out that the age difference of the experimental object was the source of heterogeneity. In subgroup analysis, the quality of research literature was high and its homogeneity was good. In order to determine the reliability and robustness of the results, the results of sensitivity analysis showed that, after eliminating single study individually, effect variable is still within 95% CI, strengthening the reliability of the original meta-analysis results. In addition, tracking the original research literature of the best effect variable gained the optimal exercise prescription for bone-invigorating: the way of exercise is brisk walking, while the exercise intensity is60%-80% HRmax, with the intervene cycle of 15 weeks and 3 times a week, each about 60 min.
