摘要
目的观察芬太尼透皮贴使用过程中执行全程疼痛管理措施对患者癌痛治疗效果的影响。方法接受芬太尼透皮贴治疗的晚期癌痛患者60例分为观察组30例和对照组30例。对照组采取入院予癌痛评估、健康宣教、发放疼痛患者教育手册、出院后电话随访等常规护理的方法,观察组采取全程疼痛管理,从入院评估到出院后1个月,按制订的计划实施操作指导、健康教育等一系列全程护理干预。观察2组干预前及干预后24 h疼痛数字分级法(NRS)评分,及治疗期间的不良反应。结果干预后24 h,2组NRS评分均较干预前降低(P <0. 05),且观察组低于对照组(P <0. 05)。观察组恶心呕吐、头晕、便秘、尿潴留的发生率均低于对照组(P <0. 05)。结论对接受芬太尼透皮贴治疗的癌痛患者应用全程疼痛管理,能提高镇痛效果,减少不良反应的发生。
Objective To investigate the effect of whole-course pain management in patients with transdermal fentanyl for cancer pain control.Methods Totally 60 patients with transdermal fentanyl for cancer pain control were assigned to the observation group (n=30) and the control group (n=30).The control group received routine nursing methods,such as cancer pain assessment,education on adverse reactions,education by handbook and telephone follow-up after discharge.The observation group adopted a series of whole-course pain control measures from admission to one month after discharge.The pain degree of patients was assessed by using Numerical Rating Scale (NRS),and adverse reactions were observed during the treatment.Results There was a decreased NRS score of cancer pain in both two groups at 24 hours after the intervention,and the NRS score was lower in the observation group than that in the control group (P<0.05).There were fewer patients with nausea and vomiting,dizziness; constipation,urinary retention in the observation group than those in the control group (P<0.05).Conclusion Whole-course pain management is effective to improve the analgesic effect and reduce risk of the adverse reaction in patients with transdermal fentanyl for cancer pain.
