摘要
目的:研究右美托咪定用于高龄髋关节置换术中辅助麻醉及镇痛的临床价值。方法:选择在我院进行髋关节置换术的高龄患者作为研究对象,随机分为接受常规全身麻醉的对照组、接受右美托咪定联合常规全身麻醉的观察组,观察手术情况并测定血清疼痛介质、炎性因子、氧化应激指标的含量。结果:两组手术时间、手术出血量、术中平均舒张压、术中心率比较无明显差异;手术中,观察组血清神经肽Y(NPY)、P物质(SP)、前列腺素E2(PGE2)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、白细胞介素-18(IL-18)、丙二醛(MDA)、促肾上腺皮质激素(ACTH)、皮质醇(Cor)、去甲肾上腺素(NE)含量明显低于对照组,超氧化物歧化酶(SOD)含量明显高于对照组(P<0.05)。结论:右美托咪定用于高龄髋关节置换术中辅助麻醉及镇痛不会影响手术情况,并且能够减轻术中疼痛、炎症及氧化应激。
Objective: To study the clinical value of dexmedetomidine for adjuvant anesthesia and analgesia in elderly patients undergoing hip replacement. Methods: The elderly patients undergoing hip replacement in our hospital between March 2015 and February 2018 were randomly divided into the control group receiving general anesthesia and the observation group receiving dexmedetomidine combined with general anesthesia. The operative conditions were observed and the serum contents of pain mediators, inflammatory factors and oxidative stress indexes were measured. Results: There was no significant difference in operation time, bleeding volume, intraoperative mean diastolic blood pressure and intraoperative heart rate between the two groups. During operation, serum contents of neuropeptide Y (NPY), substance P (SP), prostaglandin E2 (PGE2), C-reactive protein (CRP), tumor necrosis factor-α(TNF-α), interleukin-1β(IL-1β), interleukin-6 (IL-6), interleukin-18 (IL-18), malondialdehyde (MDA), adrenocorticotropic hormone (ACTH), cortisol (Cor) and norepinephrine (NE) of the observation group were significantly lower than those of the control group while serum content of superoxide dismutase (SOD) was significantly higher than that of the control group ( P<0.05). Conclusion: Dexmedetomidine for adjuvant anesthesia and analgesia in elderly patients undergoing hip replacement does not affect the operation and it can alleviate the pain, inflammation and oxidative stress during the operation.
