摘要
目的对比分析血液灌流(HP)联合血液透析(HD)治疗尿毒症皮肤瘙痒的疗效。方法将2017年2月～2018年2月期间在本院进行治疗的70例尿毒症皮肤瘙痒患者为研究对象,随机分为对照组和观察组,每组各35例。对照组患者采用HP治疗;观察组患者采用HP联合HD治疗。比较两组患者的皮肤瘙痒改善情况以及治疗前后生化指标[β2-微球蛋白(β2-MG)、甲状旁腺激素(PTH)、尿素氮(BUN)、肌酐(Cr)、血磷]水平变化情况。结果治疗后,观察组患者的皮肤瘙痒改善总有效率为94.29%,明显高于对照组,差异有统计学意义(P <0.05);治疗前两组患者的各生化指标水平比较,差异无统计学意义(P> 0.05);治疗后,观察组患者的β2-MG、PTH、Cr、血磷水平均明显低于对照组,差异有统计学意义(P <0.05);治疗后两组BUN水平比较差异无统计学意义(P> 0.05)。结论 HP联合HD治疗尿毒症皮肤瘙痒的效果显著,可有效缓解皮肤瘙痒症状,值得广泛推广使用。
Objective To compare and analyze the curative effect of hemoperfusion(HP)combined with hemodialysis(HD)in the treatment of uremic pruritus.Methods 70 patients with uremic pruritus who were treated in our hospital from February 2017 to February 2018 were selected as research objects and they were randomly divided into control group and observation group,with 35 cases in each group.Patients in the control group were treated with HP while patients in the observation group were treated with HP combined with HD.The improvement of pruritus and the changes of biochemical indicators such asβ2-microglobulin(β2-MG),parathyroid hormone(PTH),blood urea nitrogen(BUN),creatinine(Cr),serum phosphorus before and after treatment of patients in the two groups were compared.Results After treatment,the total effective rate of pruritus in the observation group was 94.29%,which was significantly higher than that in the control group.The difference was statistically significant(P<0.05).The difference in biochemical indicators before treatment between the two groups had no statistical significance(P>0.05).After treatment,β2-MG,PTH,Cr and serum phosphorus levels in the observation group were significantly lower than those in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in BUN level between two groups(P<0.05).Conclusion HP combined with HD in the treatment of uremic pruritus has significant curative effect,and it can effectively alleviate the symptoms of pruritus,which is worthy of wide promotion and application.
