摘要
目的探讨老年多发性肌炎/皮肌炎患者的临床特点。方法回顾2013年-2018年在南京医科大学附属无锡第二医院首次就诊的135例多发性肌炎/皮肌炎患者的临床资料,对其临床表现及实验室检查结果进行了分析。结果老年患者中男性的比例升高,咯痰和胸闷等非典型性临床表现发生率高,肌痛等典型临床表现发生率偏低,缺乏有效的风湿免疫等辅助检查指标,可能存在漏诊误诊;心脏损害、肺部感染、肾功能损伤和血栓栓塞等并发症的发生,导致老年患者死亡率升高。结论本研究分析了老年多发性肌炎/皮肌炎患者的临床特点,有助于提高此类患者多发性肌炎/皮肌炎的诊断率,使患者得到及时有效的治疗。
Objective To explore the clinical features of polymyositis/dermatomysitis(PM/DM)in the elderly.Methods A retrospective analysis was made to the clinical data of135cases with PM/DM during the period from2013to2018and an analysis was made to the clinical manifestations and lab test results.Results Males took up a bigger proportion in the135cases;the occurrence of atypical clinical manifestations such as expectoration and chest distress was high while the occurrence of typical clinical manifestations such as myalgia was lower;due to the lack of auxiliary index for rheumatoid immunity,possible missing or missed diagnosis might exist;the occurrence of complications including heart damage,lung infection,renal function impairment,thromboembolism led to a relatively higher death rate in the elderly.Conclusions The study made the first analysis of the clinical features of PM/DM in the elderly over60years old and is of help to raise the diagnostic rate and to provide effective and timely treatment.
