甲状腺微小癌的彩色多普勒超声诊断价值 被引量:10

摘要 目的对比甲状腺微小癌的彩色多普勒超声诊断与病理诊断结果,评估彩色多普勒超声对甲状腺微小癌的诊断价值。方法选取彩色多普勒超声诊断为甲状腺实性结节(最长径≤1 cm)并行外科手术治疗的患者100例,含106个超声诊断结节。对比彩色多普勒超声诊断和病理学诊断结果,分析彩色多普勒超声诊断甲状腺微小癌的灵敏度、特异度及准确度,评估彩色多普勒超声诊断甲状腺微小癌的临床价值。结果超声诊断结果显示:甲状腺良性结节41个,恶性结节65个。对比病理诊断结果显示,超声诊断结果符合病理诊断93个,包括34个良性结节,59个恶性结节;错误诊断13个,其中超声诊断良性病理诊断恶性7个,超声诊断恶性病理诊断良性6个。彩色多普勒超声诊断甲状腺微小癌的灵敏度、特异度、准确度分别为89.4%、85.0%、87.7%。结论彩色多普勒超声作为一种无创检查,是甲状腺微小结节良恶性鉴别诊断的首选方法,但对一些声像图特征不典型的病例,联合应用弹性成像、超声造影等补充手段,可进一步提高甲状腺微小结节诊断的准确率。 Objective To compare the ultrasonic diagnosis of thyroid microcarcinoma and pathological diagnosis,and evaluate the diagnostic value of color doppler ultrasound in thyroid microcarcinoma.Method One hundred patients with 106 solid thyroid nodules(longest diameter≤1 cm)diagnosed by ultrasound examination and underwent surgery were selected.The color doppler ultrasound diagnosis and pathological diagnosis were compared to analyze the sensitivity,specificity,accuracy of color doppler ultrasound for diagnosis of thyroid microcarcinoma and evaluate the diagnostic value of color doppler ultrasound.Result The ultrasonic diagnosis results showed that 41 benign thyroid nodules and 65 malignant thyroid nodules,compared with the pathological diagnosis results,the results of ultrasonic diagnosis were 93 nodules conformed to the pathological diagnosis,including 34 benign nodules,59 malignant nodules;there were 13 error diagnosed nodules,including 7 pathological diagnosed malignant nodules which were diagnosed benign by ultrasound and 6 pathological diagnosed benign nodules which were diagnosed malignant by ultrasound.The sensitivity,specificity and accuracy of conventional color doppler ultrasound for diagnosing thyroid microcarcinoma were 89.4%,85.0%,87.7%,respectively.Conclusion Color doppler ultrasound as a noninvasive examination,can be used as the preferred method for the differential diagnosis of the benign and malignant nodule of thyroid.For some atypical cases,conventional ultrasound combined with supplementary means such as elasticity imaging and contrast-enhanced ultrasound,can further improve the diagnostic accuracy of thyroid nodules.
