摘要
目的观察慢性阻塞性肺病急性加重期(AECOPD)患者心型脂肪酸结合蛋白(H-FABP)及心肌肌钙蛋白I(cTnI)的水平变化,探讨其在AECOPD早期心肌损伤中的应用价值。方法选取我院呼吸内科120例AECOPD患者为观察组,根据肺功能级别分为4组,每组30例。另选取60例慢性阻塞性肺病稳定期患者为对照组,采取清晨空腹血检测观察组及对照组H-FABP及cTnI水平,同时记录急性生理学和慢性健康状况Ⅱ(APACHEⅡ)评分。结果观察组患者H-FABP、cTnI、APACHEⅡ评分水平明显高于对照组,差异具有统计学意义(P <0.05)。观察组患者随着肺功能分级的增加,H-FABP、c TnI、APACHEⅡ评分也越来越高,各亚组之间比较差异具有统计学意义(P <0.05)。观察组患者血浆H-FABP水平与APACHEⅡ评分呈正相关(r=0.603,P <0.05),血浆cTnI水平与APACHEⅡ评分呈正相关(r=0.686,P <0.05)。结论 H-FABP和cTnI是判断AECOPD早期心肌损伤的良好标记物,联合APACHEⅡ评分对判断慢阻肺患者病情严重程度及预后具有重要价值。
Objective To observe the changes of heart fatty acid binding protein(H-FABP) and cardiac troponin Ⅰ( cTnI) in acute exacerbation of chronic obstructive pulmonary disease( AECOPD) patients and to evaluate the clinical value in AECOPD with myocardial injury. Methods A total of 120 patients with AECOPD were divided into 4 groups according to pulmonary function, with 30 cases in each group and 60 patients with stable COPD were selected as control group. The levels of H-FABP and cTnI in different groups were measured by fasting blood in the morning. Meanwhile, APACHE Ⅱ scores were recorded. Results H-FABP, cTnI and APACHE Ⅱ score in patients with AECOPD were significantly higher than those in the control group(P < 0.05).With the increase of pulmonary function grade, APACHE Ⅱ score,H-FABP and cTnI were also increased in the patients with AEPOCD(P < 0.05). There was a significantly positive correlation between plasma H-FABP level and APACHE Ⅱ score(r = 0.603, P < 0.05) and between plasma cTnI level and APACHE Ⅱ score(r = 0.686,P < 0.05). Conclusion H-FABP and cTnI are good markers for the diagnosis of AECOPD with myocardial injury. Combined with APACHE Ⅱ score,it has important value in judging the severity and prognosis of patients with COPD.
