尿沉渣评分系统对5 km武装越野致早期肾损伤的评估价值 被引量:1

摘要 目的寻找军事训练致肾损伤的早期敏感指标,提高预防重症肾损伤的预警能力。方法采用现况调研方法,检测425名新兵及318名老兵5 km武装越野前、运动后即刻、运动后12小时和24小时4个时间点尿液标本,观察其尿沉渣评分、尿红细胞、尿蛋白、尿微量白蛋白等指标,及在不同时间点的变化。结果训练前,尿干化学检验均为阴性,运动后即刻新兵尿沉渣评分、血尿、尿潜血、尿蛋白、尿微量白蛋白阳性率分别为31.60%、62.82%、66.35%、22.12%、36.71%,老兵分别为14.47%、30.19%、32.08%、16.35%、19.81%。以上指标新兵运动后12小时分别为7.53%、14.59%、16.71%、4.94%、10.59%,老兵分别为2.52%、4.09%、5.35%、1.89%、3.46%。P值<0.05,差异有统计学意义。运动后即刻尿评分与尿红细胞、尿潜血、尿蛋白、尿微量白蛋白秩相关系数为分别为0.530,0.492,0.806,0.758(P<0.01)。结论 5 km越野训练可导致一过性肾损伤,科学、规范的长期军事训练可减少肾损伤,尿沉渣评分系统在一定程度上可作为评估训练后肾损伤的参考指标。 Objective To find early sensitive index of renal injury caused by military training,so as to improve the early warning ability of preventing severe renal injury.Methods By using the method of current researches,detecting four time points urine samples of425recruits and318veterans before,immediately,and12hours and24hours after the5km armed cross-country,detecting indicators of urinary sediment score,urinary red blood cells,urine protein and urinary albumin,etc.,observing the changes at different time points were taken.Results Before training,urine dry chemistry test was negative.Immediately after training,urinary sediment score,hematuria,urinary protein,hemoglobin,urinary albumin positive rate of recruit was31.60%,62.82%,66.35%,22.12%,36.71%,and veterans was14.47%,30.19%,32.08%,16.35%,19.81%(P<0.05)respectively.At12hours after training,the above indexes of recruit were7.53%,14.59%,16.71%,4.94%,10.59%,and the above indexes of recruit veterans were2.52%,4.09%,5.35%,1.89%,3.46%,respectively;the value of P was all less than0.01.Immediately after training,the coefficient respectively between urine score and red blood cells,urine occult blood,urine protein,urine white protein were0.530,0.492,0.806,0.758(P<0.01)respectively.Conclusion5km cross-country training can lead to temporal renal injury,but scientific,standardized long-term military training can reduce renal one,and urinary sediment scoring system may be used to evaluate the renal injury after training.
