摘要
目的通过对某三甲医院食管癌手术患者住院费用影响因素进行研究,为DRGs(Diagnosis Related Groups system)疾病诊断相关组法支付方式提供依据。方法收集某三甲医院食管癌手术患者的基本信息资料,采用单因素分析法、结构方程模型对食管癌手术患者的住院费用进行分析,找出住院费用的影响因素及其之间的相互关系。结果食管癌手术患者的住院费用影响因素有年龄、费别、手术方式、合并症、并发症、院感、转科、术前住院天数、住院天数,其中住院天数是影响住院费用的最重要因素。结论有效缩短住院天数,减少不必要的医疗行为,利于降低手术患者住院费用。
Objective To provide basis for DRGs payment methods with the study of influential factors of patients with esophageal cancer surgery in a first-class hospital.Methods The influence factors of hospitalization expenses and the relationships between them had been found out with the analysis of the hospitalization expenses of patients with esophageal cancer surgery after collecting basic information with esophageal cancer surgery from a first-class hospital.Single-factor analysis and structural Equation Model were used in this paper.Results The influence factors of hospitalization expenses of patients with esophageal cancer surgery include age,cost burden,surgical procedure,comorbidities,complications,hospital sensation,transfer,preoperative hospital stay and hospital stay,etc.in which the length of hospital stay was the most important factor affecting hospitalization expenses.Conclusion Shortening the length of stay and reducing unnecessary medical treatment will help reduce the cost of hospitalization for surgical patients.
