摘要
目的:探讨食管癌术后胸腔引流管拔除时机。方法:回顾性分析2019年12月-2021年3月山东第一医科大学附属肥城医院手术治疗的120例食管癌患者资料,根据术后胸腔引流量分为A、B两组。A组术后胸腔引流量<100 mL/d拔管,B组术后胸腔引流量100~250 mL/d拔管。对比两组患者引流管留置时间以及引流量、术后恢复情况、拔管疼痛情况。记录两组患者拔管后不良反应的发生情况。结果:B组引流管留置时间以及住院时间均较A组短(P<0.05),B组平均引流量高于A组(P<0.05)。拔管后B组疼痛视觉模拟(VAS)评分明显低于A组(P<0.05)。两组拔管后并发症发生率比较,差异无统计学意义(P>0.05)。结论:食管癌开胸术后患者,胸腔引流量100~250 mL/d拔除胸腔引流管,引流管留置时间和住院时间显著优于胸腔引流量<100 mL/d,且并不增加术后并发症。
Objective:To explore the timing of removal of thoracic drainage tube after esophageal cancer surgery.Method:The data of 120 patients with esophageal cancer treated by surgery in Feicheng Hospital Affiliated to Shandong First Medical University from December 2019 to March 2021 were analyzed retrospectively,according to the postoperative thoracic drainage volume,they were divided into group A and group B.The postoperative thoracic drainage volume in group A was less than 100 mL/d,and the postoperative thoracic drainage volume in group B was 100-250 mL/d.The drainage tube indwelling time,average drainage volume,postoperative recovery and pain of extubation were compared.The incidences of adverse reactions after extubation were recorded in both groups.Result:The drainage tube indwelling time and hospital stay in group B were shorter than those in group A(P<0.05),and the average drainage volume in group B was higher than that in group A(P<0.05).The VAS score of group B was significantly lower than that of group A after extubation(P<0.05).There was no significant difference in the incidence of complications after extubation between the two groups(P>0.05).Conclusion:In patients with esophageal cancer after thoracotomy,the thoracic drainage tube is removed with a thoracic drainage volume of 100-250 mL/d,the indwelling time and hospital stay of the drainage tube are significantly better than those with a thoracic drainage volume of less than 100 mL/d,and postoperative complications are not increased.
