摘要
目的探讨安宁疗护模式对结肠癌晚期患者生理、心理、精神以及社会层面的作用效果。方法筛选2016年4月至2017年2月收治的结肠癌临终患者56例,采用随机数字表法将入组患者分为对照组和干预组各28例。护理期间,给予对照组常规护理,干预组在此基础上进行安宁疗护,干预前后采用问卷对两组患者进行评价,比较两组患者的生活质量及患者家属满意度。结果干预后,干预组患者QLQ-C30量表中总体健康状况、功能子量表及症状子量表中疲乏、疼痛、恶心呕吐、食欲丧失、便秘得分与干预前相比,差异均有统计学意义(p<0.05),而症状子量表中呼吸困难、失眠、腹泻得分差异均无统计学意义(p>0.05);对照组患者QLQ-C30量表及各维度得分干预前后差异无统计学意义(p>0.05),干预组患者家属的满意度高达100%。结论对于结肠癌晚期患者,实行安宁疗护可使患者及家属得到多方位的照顾,促进患者癌症晚期的生活质量,提高患者家属满意度,可尝试在晚期癌症患者中推广应用。
Objective To explore the effects of hospice care mode on the physiological,psychological,spiritual and social aspects of patients with advanced colon cancer.Method A total of 56 terminal stage patients with colon cancer admitted from April 2016 to February 2017 were enrolled.The patients were divided into control group(n=28)and intervention group(n=28).During the care,the control group was given routine nursing care.The intervention group was given hospice care on the basis of control group.The questionnaires were used to evaluate the two groups of patients before and after the intervention.The quality of life and the satisfaction of the patients of the two groups were compared.Results Compared with before the intervention,the overall health status,and scores of fatigue,pain,nausea,vomiting,loss of appetite,constipation in function subscale and symptom subscale after the intervention in the intervention group were significantly different(p<0.05).The scores of dyspnea,insomnia and diarrhea in the symptom subscale were no statistically different before and after the intervention in the intervention group(p>0.05).In the control group,there was no statistical difference in the total score of QLQ-C30 scale and the scores of each dimension before and after intervention(p>0.05).The satisfaction rate of the family members in the intervention group was as high as 100%.Conclusion For patients with advanced colon cancer,hospice care can provide patients and their families with multi-faceted care,promote the quality of life,and improve the satisfaction of patients'family.Hospice care can be promoted in patients with advanced cancer.
