老年帕金森病患者血浆同型半胱氨酸水平与认知障碍相关性研究 被引量:2

Correlation Between Plasma Homocysteine Level and Cognitive Impairment in Elderly Patients with Parkinson's Disease

摘要 目的探讨分析老年帕金森病患者血浆同型半胱氨酸水平与认知障碍相关性。方法方便选取该院自2017年1—12月收治的60例老年帕金森病患者作为临床研究对象,其中认知障碍的有30例,纳入认知障碍组;认知正常的有30例,纳入认知正常组;另选取同期到我院进行健康体检的健康老年人30名作为对照,纳入健康组;回顾分析两组老年帕金森病患者的基本资料,检测比较3组的血浆同型半胱氨酸水平、叶酸水平及维生素B12水平,分析MMSE评分与3项指标的相关性。结果认知障碍组的MMSE评分[(16.03±4.06)分]显著低于认知正常组[(22.95±3.38)分](P<0.05),病程[(7.3±2.9)年]显著长于认知正常组[(2.2±0.8)年]差异有统计学意义(P<0.05)。认知障碍组和认知正常组的血浆同型半胱氨酸水平均[(28.72±6.13)μmol/L、(19.29±5.36)μmol/L]显著高于健康组[(9.73±2.11)μmol/L]差异有统计学意义(P<0.05);认知障碍组的血浆同型半胱氨酸水平显著高于认知正常组(P<0.05)。3组的叶酸水平和维生素B12水平比较差异无统计学意义(P>0.05)。血浆同型半胱氨酸水平与MMSE评分存在明显的负相关(r=-0.689,P<0.05),其他几项指标则与MMSE评分不存在明显的相关性(P>0.05)。结论血浆同型半胱氨酸水平与认知障碍存在明显的相关性,血浆同型半胱氨酸水平升高者更易出现认知障碍,临床应加强对老年帕金森病患者血浆同型半胱氨酸水平的监测,以预防认知障碍。 Objective To investigate the relationship between plasma homocysteine level and cognitive impairment in elderly patients with Parkinson's disease.Methods A total of 60 elderly patients with Parkinson's disease admitted to the hospital from January 2017 to December 2017 were convenient selected and enrolled as clinical subjects.Among them,30 were cognitive impairment patients,and 30 were cognitively normal,included in the normal cognitive group;another 30 healthy elderly patients who were admitted to the hospital for health examination in the same period as comparison,included in the health group;retrospective analysis of the basic data of the two groups of elderly patients with Parkinson's disease,the detection of three groups of plasma isotype Cystine levels,folate levels,and vitamin B12 levels were analyzed for correlation between MMSE scores and three indicators.Results The MMSE score of the cognitive impairment group[(16.03±4.06points)]was significantly lower than that of the normal cognitive group[(22.95±3.38points)](P<0.05),and the course of disease[(7.3±2.9)years]was significantly longer than the cognitive normal group[(2.2±0.8)years](P<0.05).Plasma homocysteine levels in the cognitive impairment group and the cognitive normal group[(28.72±6.13)μmol/L,(19.29±5.36)μmol/L]were significantly higher than those in the healthy group[(9.73±2.11)μmol/L],the different was statistically significant(P<0.05);plasma homocysteine level in the cognitive impairment group was significantly higher than that in the cognitive normal group(P<0.05).There were no significant differences in folic acid levels and vitamin B12 levels between the three groups,the different was not statistically significant(P>0.05).There was a significant negative correlation between plasma homocysteine level and MMSE score(r=-0.689,P<0.05).There were no significant correlations between other indexes and MMSE scores(P>0.05).Conclusion There is a significant correlation between plasma homocysteine levels and cognitive impairment.Patients with
