经皮穴位电刺激联合膀胱功能训练对脊髓损伤后神经源性膀胱排尿功能的影响 被引量:8

Effect of Percutaneous Acupoint Electrical Stimulation Combined with Bladder Function Training on Neurogenic Bladder Urination after Spinal Cord Injury

摘要 目的探讨经皮穴位电刺激联合膀胱功能训练对脊髓损伤后神经源性膀胱患者排尿功能的影响。方法将86例脊髓损伤后神经源性膀胱患者按"不平衡指数最小"分配原则分为对照组和观察组各43例,2组患者均接受脊髓损伤恢复期的常规康复治疗及对各并发症给予对症处理,针对神经源性膀胱,对照组采用间歇导尿、手法排尿训练及盆底肌功能训练,观察组患者在此基础上同时予经皮穴位电刺激治疗,8周后比较2组疗效。结果治疗后,2组的24 h平均单次尿量、24 h单次最大尿量均显著增加(P<0.05),24 h平均排尿次数、24h平均尿失禁次数均较治疗前显著减少,且治疗后观察组各指标改善水平优于对照组(P<0.05)。治疗后2组患者的最大膀胱容积和最大尿流速率则有明显上升,而残余尿量均有明显下降(P<0.05);且治疗后观察组的最大膀胱容积和最大尿流速率均高于对照组,而残余尿量低于对照组(P<0.05)。观察组的总有效率为93.02%(40/43),高于对照组的81.40%(35/43)(P<0.05)。结论在常规康复训练的基础上,经皮穴位电刺激联合膀胱功能训练能进一步改善脊髓损伤患者的膀胱排尿功能。 Objective To investigate the effect of percutaneous acupoint electrical stimulation combined with bladder function training on urination function of neurogenic bladder patients after spinal cord injury.Methods 86 patients with neurogenic bladder after spinal cord injury were divided into control group(n=43)and observation group(n=43)according to the principle of"minimum imbalance index".The patients in both groups received routine rehabilitation and symptomatic treatment of the complications during the recovery period of spinal cord injury.For the neurogenic bladder,the control group were treated with intermittent catheterization,manual urination training and pelvic floor muscle function training.On this basis,the patients in the observation group were treated with percutaneous acupoint electrical stimulation for 8 weeks and the therapeutic effects were compared between the two groups.Results After treatment,the mean single time urine volume and the maximum volume of urine at 24 hours in both groups were significantly increased(P<0.05),and the mean urinary incontinence times in 24 hours and 24 hours after treatment were significantly decreased compared with those before treatment.After treatment,the improvement level of each index in the observation group was better than that in the control group(P<0.05).After treatment,the maximal bladder pressure measurement volume and the maximum urinary flow rate of the two groups increased significantly,while the residual urine volume decreased significantly(P<0.05),and the maximum bladder pressure measurement volume and the maximum urinary flow rate in the observation group were higher than those in the control group after treatment.The residual urine volume was lower than that in the control group(P<0.05).The total effective rate of the observation group was 93.02%(40/43),which was higher than that of the control group(81.40%(35/43)(P<0.05).ConclusionOn the basis of routine rehabilitation training,percutaneous acupoint electrical stimulation combined with bladder function
