动态对比增强磁共振成像在脑胶质瘤患者瘤细胞增殖定量评估中的应用 被引量:6

摘要 目的探讨动态对比增强磁共振成像(DCE-MRI)在脑胶质瘤患者瘤细胞增殖定量评估中的应用价值。方法 79例脑胶质瘤患者根据2007版世界卫生组织(WHO)胶质瘤分级标准分为Ⅱ级30例[低级别胶质瘤(LGG)]和Ⅲ级8例、Ⅳ级41例[高级别胶质瘤(HGG)],所有患者术前均行DCE-MRI检查,测算容积转运参数(K^(trans))、血管外细胞外间隙容积比(Ve)的最大值;术后对标本进行免疫组化检测,测算Ki-67增殖指数最大值。应用Mann-Whitney U检验比较不同级别胶质瘤K^(trans)、Ve、Ki-67增殖指数的差异。应用Spearman相关分析法分析K^(trans)、Ve与Ki-67增殖指数的相关性。结果与LGG比较,HGG的K^(trans)、Ve、Ki-67增殖指数增加(P均<0. 05);HGG的K^(trans)、Ve与Ki-67增殖指数均呈正相关(r分别为0. 614、0. 644,P均<0. 05),LGG的K^(trans)、Ve与Ki-67增殖指数均无相关性(P均> 0. 05)。结论 DCE-MRI可以定量评估HGG患者瘤细胞增殖程度。 Objective To quantitatively assess the tumor cell proliferation of brain gliomas with the volume transfer constant(K trans)and volume of extravascular extracellular space per unit volume of tissue(Ve)derived from dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI).Methods The preoperative DCE-MRI data of 79 patients with pathologically confirmed glioma were retrospectively reviewed.According to the 2007 World Health Organization(WHO)classification,the pathological evaluation revealed that 30 patients had gradeⅡglioma(low grade glioma,LGG),8 with gradeⅢand 41 with gradeⅣ(high grade glioma,HGG).All patients underwent DCE-MRI preoperatively.The maximal values of K trans and Ve were measured from each patient.The maximal Ki-67 proliferation index of each tumor was measured in surgical specimens with the immunohistochemistry after operation.The Mann-Whitney U test was applied to analyze the differences in K trans,Ve,and Ki-67 proliferation indexes across the different grades of glioma.Spearman correlation was performed between K trans,Ve,and Ki-67 proliferation index.Results K trans,Ve,and Ki-67 proliferation indexes of HGG were significantly higher than those of LGG(P<0.05).Both K trans and Ve were significantly correlated with Ki-67 proliferation index in HGG(r=0.614,0.644;both P<0.05).But there were no correlations between K trans,Ve and Ki-67 indexes in LGG(all P>0.05).Conclusion DCE-MRI can be used to assess the tumor cell proliferation in HGG quantitatively.
