摘要
目的:分析老年煤工尘肺(CWP)患者C-反应蛋白(CRP)水平变化对疾病严重程度、肺功能疾病的诊断与防治的意义。方法:收治老年CWP患者58例,同期选取健康者32例作对照组。测定不同分期老年CWP患者与健康者C-反应蛋白水平与肺功能指标水平,判定C-反应蛋白水平与患者肺功能、疾病感染严重程度间的关系。结果:老年CWPⅠ期、Ⅱ期、Ⅲ期患者的CRP水平都显著高于对照组;肺功能指标水平(FEV1、VC、FVC)均显著低于对照组;CWPⅡ期、Ⅲ期患者CRP水平显著高于Ⅰ期,肺功能指标水平显著低于CWPⅠ期;CWPⅢ期患者CRP水平显著高于Ⅱ期;肺功能指标水平显著低于CWPⅡ期。CWP合并肺部感染老年患者的CRP明显高于CWP合并肺气肿及慢性阻塞性肺疾病老年患者,差异均有统计学意义(P<0.05)。结论:随着老年CWP病情加重,其肺功能损伤程度不断加重,炎性反应更显著。老年CWP患者C-反应蛋白水平能反应患者疾病严重程度、肺功能指标变化与肺功能损伤情况,同时对于合并其他肺疾病的判定有指导意义。
Objective:To analyze the significance of changes of C-reactive protein(CRP)level in elderly coal workers pneumoconiosis(CWP)to the diagnosis and prevention and treatment of disease severity and pulmonary function diseases.Methods:58 elderly patients with CWP were selected,and 32 healthy cases were selected as the control group,we determined the level of C-reactive protein and lung function in elderly CWP patients and healthy people with different stages,the relationship between Creactive protein level and lung function and severity of disease infection was determined.Results:CRP levels in elderly patients with CWPⅠ,stageⅡandⅢwere significantly higher than those in the control group.The level of pulmonary function(FEV1,VC,FVC)was significantly lower than that of the control group.The CRP levels of patients with stageⅡandⅢCWP were significantly higher than those of stageⅠ,and the level of lung function was significantly lower than that of stage CWPⅠ.CRP levels in patients with CWP stageⅢwere significantly higher than those in stageⅡ,pulmonary function index level was significantly lower than CWPⅡstage.CRP in elderly patients with CWP complicated with pulmonary infection was significantly higher than that in elderly patients with CWP complicated emphysema and chronic obstructive pulmonary disease,the differences were statistically significant(P<0.05).Conclusion:With the aggravation of CWP in elderly patients,the degree of lung function damage is aggravating,and the inflammatory reaction is more significant.The level of C-reactive protein in elderly CWP patients can reflect the severity of the disease,the change of lung function and the damage of lung function,and it is of guiding significance for the determination of other pulmonary diseases.
