矽肺患者自身免疫指标检测意义的研究 被引量:7

Study on the significance of autoimmune indexes in patients with silicosis

摘要 目的探讨矽肺患者血清中3项自身免疫指标的表达水平和(或)异常率,以及与矽肺患者肺部病变程度的相关性。方法以69名矽肺患者为病例组,69名健康人作为对照组。分别采用免疫比浊法、间接酶联免疫吸附法、间接免疫荧光法对两组人群血清中的类风湿因子(RF)、抗双链DNA(ds-DNA)抗体的表达水平及ANA的异常率进行检测;对病例组进行血气分析、肺通气功能检测,并对结果进行比较和相关性分析。结果病例组血清中的ANA异常率高于对照组(P<0.05);病例组与对照组血清中的RF、抗ds-DNA抗体表达水平均在正常范围内,病例组RF及抗ds-DNA抗体的表达水平与对照组比较,差异无统计学意义(P>0.05);不同分期矽肺患者间血气分析检测结果比较,差异无统计学意义(P>0.05);矽肺叁期患者肺通气功能较壹、贰期患者下降明显(P<0.05),矽肺壹期与贰期患者间肺通气功能比较,差异无统计学意义(P>0.05);ANA在贰期矽肺、肺区小阴影总体密集度为2级等情况下异常率最高,但在不同的矽尘接触时间、矽肺分期、肺区总体密集度、肺通气功能及血气分析指标氧分压正常与否间的异常率比较,差异无统计学意义(P>0.05);矽肺贰期患者血清中的RF、抗ds-DNA抗体表达水平最高,但在不同分期矽肺患者间血清中的RF、抗ds-DNA抗体表达水平比较,差异无统计学意义(P>0.05);矽肺患者血清中的RF、抗ds-DNA抗体表达水平与患者年龄、矽尘接触时间、肺通气功能、血气分析、矽肺诊断分期及肺区总体密集度不存在相关性(P>0.05)。结论矽肺患者血清中的ANA异常率较正常人高,RF、抗ds-DNA抗体无出现高表达;三者与矽肺患者肺部病变程度不存在相关性。ANA可能与矽肺发病机理有关,其临床意义需进一步研究。 Objective To investigate the expression of three autoimmune indexes in silicosis patients:the levels of rheumatoid factor(RF),anti ds-DNA antibody(anti ds-DNA)and the abnormal rate of antinuclear antibody(ANA);And to explore their correlation with the degree of lung lesions.MethodsSixty-nine patients with silicosis were chosen as the case group.Sixty-nine healthy people were selected as the control group.The levels of RF,anti ds-DNA antibody and the abnormal rate of ANA in serum of two groups were detected by immunoturbidimetry technique,indirect enzyme-linked immunosorbent assay technique and indirect immunofluorescence respectively technique.Blood gas analysis and pulmonary ventilation function were performed in the case group,and the results were compared and analyzed the correlativity.Results The abnormal rate of ANA in the case group was higher than which in the healthy control group(P〈0.05);the expression levels of RF and anti ds-DNA antibodies in the serum of case group and control group were both within the normal range,and RF and anti ds-DNA antibodies levels were not significantly different from healthy controls(P〉0.05); There were no significant differences in blood gas analysis between different stages of silicosis patients(P〉0.05);The pulmonary ventilation function of silicosis patients in stageⅢ was significantly lower than which of stageⅠ andⅡ(P〈0.05),but there was no significant difference between the stage Ⅰ and Ⅱ patients(P〉0.05);In the case that the small shadow of the general dispersion in pulmonary area was grade 2or in stageⅡ of silicosis,the abnormality rate of ANA was the highest,but there were no significant differences in the abnormal rate of ANA in the different dust-exposing time,silicosis stages,general dispersion in pulmonary area,and whether the pulmonary ventilation functions and oxygen partial pressure of blood gas analysis indexes were normal or not(P〉0.05).The levels of RF and anti ds-DNA antibodies were highest in si
