摘要
目的:探讨高通量透析对尿毒症合并继发性甲状旁腺功能亢进(SHPT)患者甲状腺钙磷水平及甲状腺旁素的影响。方法:收治尿毒症合并SHPT患者110例,随机分为两组。对照组给予F6低通量血液透析治疗,观察组给予F60高通量透析治疗,对比两组血钙、血磷、碱性磷酸酶(AKP)、iPTH、骨性AKP水平及生存率。结果:观察组透析后血磷、iPTH、骨性AKP水平低于透析前,且低于对照组,差异有统计学意义(P<0.05)。观察组生存率显著高于对照组,差异有统计学意义(P<0.05)。结论:高通量透析可有效改善尿毒症合并SHPT患者的磷代谢紊乱,降低iPTH水平。
Objective:To investigate the effect of high flux hemodialysis on the thyroid calcium and phosphorus levels and parathyroid hormone in patients with uremia complicated with secondary hyperparathyroidism(SHPT).Methods:110 cases of patients with uremia combined with SHPT were divided into two groups randomly.The control group was treated with F6 low flux hemodialysis,and the observation group was treated with F60 high flux hemodialysis,the levels of blood calcium,blood phosphorus,alkaline phosphatase(AKP),iPTH,bone AKP and survival rate were compared between the two groups.Results:Tthe levels of serum phosphorus,iPTH and bone AKP in the observation group after dialysis were lower than predialysis,and were lower than those in the control group,and the difference was statistically significant(P<0.05).The survival rate of the observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:High flux hemodialysis could improve the phosphorus metabolism disorder effectively and reduce the level of iPTH in patients with uremia complicated with SHPT.
