摘要
目的:探讨中药坐浴联合改良RPH术在混合痔患者治疗中的应用。方法:选取100例混合痔患者,随机数字表法分为两组。对照组予改良RPH术治疗,观察组予中药坐浴(枯矾、海螵蛸、冰片、当归、赤芍、丹皮、黄芩、黄连、大黄、五倍子等)结合改良RPH术治疗。对比两组疗效、手术相关并发症、肛管直径及肛管压力。结果:观察组总有效率98%显著高于对照组86%(χ~2=4.62,P<0.05);观察组愈合及住院时间、疼痛及出血积分、创面水肿情况均显著优于对照组(t=4.83、5.73、5.21、3.82、4.72,P<0.05);观察组不良并发症总发生率12%显著低于对照组36%(χ~2=6.84,P<0.05)。两组肛管直径、肛管静息压均较术前显著下降(t=5.21、4.18、4.62、5.23,P<0.05),且观察组肛管直径、肛管静息压均显著低于对照组(t=3.64、5.12,P<0.05),而两组肛管收缩压较术前及组间比较均无显著差异(t=0.96、0.85、0.67,P>0.05)。结论:中药坐浴结合改良RPH术对混合痔患者临床疗效显著,有效缓解患者痛苦、缩短住院时间,且并发症少。
Objective:To explore the application of modified Chinese medicine combined with RPH in the treatment of mixed hemorrhoids.Methods:A total of 100 mixed hemorrhoids patients were selected,and the random number table method was divided into two groups.The control group was treated with modified RPH.Observation group the main medicine of traditional Chinese medicine sitting bath(alum,sepium,ice sheet,angelica,radix paeoniae,danpi,scutellaria baicalensis,huanglian,rhubarb,gallas,etc),Combined with modified RPH.Systematic observation efficacy,surgical related conditions including complications,anal diameter and anal pressure.Results:The total effective rate of the observation group was 98%significantly higher than that of the control group 86%(χ2=4.62,P<0.05).The healing and hospitalization time of the observation group,the pain and bleeding score,the edema of the wound was significantly better than the control group(t=4.83、5.73、5.21、3.82、4.72,P<0.05).The total incidence of adverse complications in the observation group was 12%lower than that of the control group 36%(χ2=6.84,P<0.05).Two groups of anal canal diameter,anal canal resting pressure than preoperative declined significantly(t=5.21、4.18、4.62、4.18,P<0.05),and anal canal diameter,anal canal resting pressure observation group was significantly lower than the control group(t=3.64、5.12,P<0.05),and two groups of anal canal systolic blood pressure had no significant difference was compared between the preoperative and group(t=0.96、0.85、0.67,P>0.05).Conclusion:The treatment of mixed hemorrhoids with Chinese herbal medicine sitting bath combined with improved RPH is effective in treating mixed hemorrhoids,alleviating patient's pain effectively,shortening the time of hospitalization,and less complications.
