微创经皮椎弓根钉内固定结合椎体成形术治疗老年胸腰椎爆裂性骨折疗效分析 被引量:5

摘要 目的探讨微创经皮椎弓根钉结合椎体成形术(pereutaneous vertebroplasty,PVP)治疗老年胸腰椎爆裂性骨折的临床疗效。方法回顾性分析30例微创经皮椎弓根钉结合PVP术治疗老年胸腰椎爆裂性骨折,观察术前、术后1个月和末次随访椎体后凸角(Cobb角),椎体前、后缘高度及疼痛视觉模糊评分(VAS)。结果术后1个月和末次随访时Cobb角,椎体前、后缘高度、VAS评分与术前比较,差异均有统计学意义(P<0.01)。结论微创经皮椎弓根钉内固定结合PVP术治疗老年无神经症状胸腰椎爆裂性骨折创伤小,出血少、可早期下床活动,降低了老年患者长期卧床带来的各种并发症,为老年胸腰椎爆裂性骨折提供了一种安全有效的微创治疗方法。 Objective To investigate the clinical efficacy of minimally invasive percutaneous pedicle screw combined with vertebroplasty(PVP)in the treatment of thoracolumbar burst fractures in the elderly.Methods 30 cases of senile thoracolumbar burst fractures treated with minimally invasive percutaneous pedicle screw and PVP were retrospectively analyzed.The kyphosis(Cobb angle),height of anterior and posterior edges of vertebral body and visual blur score(vas)of pain were observed before and 1 month after operation and the last follow-up.Results Cobb angle,vertebral body anterior and posterior edge height and vas scores at 1 month after surgery and at the last follow-up were statistically significant compared with those before surgery(P<0.01).Conclusion Minimally invasive percutaneous pedicle screw fixation combined with PVP is a safe and effective minimally invasive treatment method for elderly patients with thoracolumbar burst fractures without neurological symptoms.It has less trauma,less bleeding and can move out of bed early.it reduces all kinds of complications caused by long-term bed rest of elderly patients and provides a safe and effective minimally invasive treatment method for elderly patients with thoracolumbar burst fractures.
