摘要
目的比较采用自体腘绳肌腱(HT)与部分腓骨长肌腱(PLT)进行前交叉韧带(ACL)单束重建的疗效。方法选取采用HT与部分PLT进行ACL重建手术的患者127例,根据移植物不同,分为HT组79例和部分PLT组48例。术前及术后6、12个月门诊复诊时采用疼痛视觉模拟评分(VAS)评估患者疼痛情况,采用Lysholm评分和Tegner评分评估患者膝关节运动功能,并测量膝关节活动度,进行组间比较。结果两组患者术后6、12个月VAS评分均明显低于术前,Lysholm评分和Tegner评分均明显高于术前,膝关节活动度均较术前明显改善,差异均有统计学意义(均P<0.05)。术后6个月部分PLT组Lysholm评分优于HT组,差异有统计学意义(P<0.05);其余各指标不同时点两组的差异均无统计学意义(均P>0.05)。结论采用部分PLT重建ACL疗效较为确切,术后患者疼痛状况、膝关节功能及活动度改善情况与临床常用的自体HT相仿,是值得尝试的一种ACL重建移植物。
Objective To compare the clinical outcomes of anterior cruciate ligament(ACL)reconstruction with autologous hamstring tendon(HT)and partial peroneus longus tendon(PLT). Methods 127 patients undergoing reconstruction of ACL were enrolled in the study,HT was used for reconstruction in 79 cases(HT group)and partial PLT was used in 48 cases(PLT group).The pain visual analogue scale(VAS)was used to evaluate the pain before and 6 and 12 months after the operation.The Lysholm score and Tegner score were used to evaluate the knee joint motor function,and the knee joint mobility was measured.The VAS score,Lysholm score,Tegner score,and knee joint mobility were compared before and 6 and 12 months after operation between the two groups. Results After reconstruction of ACL,the VAS score was decreased significantly,the Lysholm score and Tegner score were increased,and the knee joint mobility was improved at 6 and 12 months after operation in both groups(all P<0.05).The Lysholm score of the PLT group was better than that of the HT group at 6 months after operation(P<0.05);however,there was no significant difference between the two groups at other time points(all P>0.05). Conclusion Reconstruction of ACL with autologous partial PLT has similar clinical outcomes in the improvement of pain status,knee function and the knee joint mobility as those with autologous HT graft.
