围手术期人文关怀对心脏外科患者就医总体满意度的影响 被引量:5

The influence of humanistic care in perioperative period on the overall satisfaction of cardiac surgery patients

摘要 目的探讨手术室护士对心脏外科患者实施围手术期人文关怀对患者就医总体满意度的影响,为优化心脏外科手术患者护理方案,提升护理质量提高参考。方法选取2016年1—12月收治的228例心脏外科手术患者为对照组,2017年1—12月收治的238例心脏外科手术患者为观察组。对照组患者实施常规围手术期护理,观察组由经过人文关怀护理培训合格的手术室护士在对患者实施常规围手术期护理过程中严格执行人文关怀护理理念。采用焦虑自评量表(SAS)和抑郁自评量表(SDS)对两组患者入组后即刻、出院前情绪状态进行测评,采用数字评定量表(NRS)对患者麻醉苏醒后1,24,72 h疼痛程度进行测评,采用Kolcaba舒适状况量表对患者完成手术苏醒后、术后3 d、出院前的舒适度测评,采用我院自制护理满意度调查表患者对就医总体满意度进行调查。结果两组患者入组后即刻SDS,SAS评分比较差异无统计学意义(P>0.05),出院前两组患者SDS,SAS评分均较入组后即刻有所降低,且观察组均低于对照组(P<0.05)。观察组患者术后24,72 h NRS疼痛评分均明显低于对照组(P<0.05)。观察组患者手术苏醒后、术后3 d、出院前各项Kolcaba舒适度评分均明显高于对照组(P<0.05)。观察组患者就医总体满意度明显高于对照组(P<0.05)。结论手术室护士将人文关怀理念融入到心脏外科手术患者的围手术期护理中,可有效增强患者身心舒适度,提升患者就医总体满意度,具有较高的临床价值。 Objective To optimized the nursing program of patients undergoing cardiac surgery and improved the quality of nursing,the influence of perioperative humanistic care by nurses in operating room on the overall satisfaction of cardiac surgery patients was studied.Methods To evaluated the emotional state of immediate admission and before discharge with self-rating anxiety scale(SAS)and self-rating depression scale(SDS);to evaluated the degree of the pain of1h,24h,and 72h after anesthesia recovery with number rating scale(NRS);to observed the comfort status of after surgery,3days after surgery,and before discharge with Kolcaba general comfort questionnaire;and to investigated the overall medical satisfaction by using self-designed questionnaire of nursing satisfaction in our hospital,466 cases of cardiac surgery patients in total were observed and studied.Particularly,228 patients treated in our hospital from January to December 2016 were sampled as the study group,while the 238 cardiac surgery patients admitted from January to December 2017 were sampled as the control group.The control group was given the routine perioperative nursing,while the study group was given the strict humanistic care nursing implemented by nurses in the operating room,who had been trained and qualified in the humanistic care nursing cultivation.Results There was no statistical significance in the difference of the scores of immediate SDS and SAS between the two groups after admission(P>0.05).Yet,the scores of SDS and SAS of the two groups before discharge were slightly lower than those after admission,and the scores of the study group were generally lower than that of the control group(P<0.05).The NRS pain scores of 24h and 72 after surgery in the study group were apparently less than that in the control group(P<0.05).Besides,the scores of the Kolcaba general comfort of after surgery,3days after surgery,and before discharge of the study group were significantly higher than those of the control group(P<0.05).The overall satisfaction of the st
