不同进餐持续时间对糖尿病患者血糖水平和胰岛素分泌的影响 被引量:2

摘要 目的拟以标准热卡早餐为前提,观察进餐持续时间的长短对糖尿病患者的血糖和胰岛素分泌所产生的影响,以更好地指导糖尿病患者的饮食,为糖尿病的综合防治提供基础数据。方法选择我院2013年8月~2016年2月收治的糖尿病患者31例,采取自身前后对照的方法,对31例2型糖尿病患者予标准热卡的早餐,第1、2天,要求受试者在30分钟内把早餐进食完毕;第3、4天,要求受试者的进食时间≥30分钟,于第2、第4天抽取空腹及进食第一口早餐后30、120、180、240分静脉血检测胰岛素值,同时扫描FSL获取瞬时血糖值,进行对比、分析。结果前后自身对照短时长和长时长进餐,发现短时长进餐血糖和胰岛素峰值均于餐后120min出现,随后逐渐下降;长时长进餐血糖和胰岛素虽然同样于餐后120min出现,但高峰一直延续到餐后180min后才缓慢下降,餐后240min仍处于较高水平;短时长和长时长进餐餐后240min血糖值分别为(25.46±23.00)μU/mL和(40.25±41.34)μU/mL,差异有统计学意义(P<0.05);其余葡萄糖及胰岛素曲线下面积、胰岛β细胞功能指数差异无统计学意义(P>0.05)。结论在进餐量相对固定的情况下,早餐进餐时间延长,增加餐后胰岛素分泌持续时长。 Objective On the premise of standard calorie breakfast,to observe the influence of duration time of different meals for blood glucose level and insulin secretion in patients with diabetic,and to better guide the diet of diabetic patients and provide basic data for the comprehensive prevention and treatment of diabetes.Methods Thirty-one cases of patients with diabetic and who treated in our hospital from August 2013 to February 2016 were selected.And take the method of comparing itself before and after,31 cases of patients with type 2 diabetic were given the standard calorie breakfast,on the first,second day,the subjects were asked to finish their breakfast in 30 minutes.Third,fourth days,participants were asked to eat more than 30 minutes of time.On the second and fourth days,the empty stomach,30,120,180 and 240 minutes after the first breakfast was taken to detect the insulin value.At the same time,the FSL was scanned to obtain the instantaneous blood glucose value,and the comparison and analysis were made.Results In the short and long time meal before and after the self control,the blood glucose and insulin peak at short time meal were found to appear at postprandial 120min,then gradually decreased.In the long time meal,blood sugar and insulin were although found to appear the same as the 120min after the meal,but the peak continued until after the meal 180min after the slow decline,after the meal was still at a high level of 240min.The blood sugar value in 240min after a short and long time meal respectively were(25.46±23.00)μU/mL and(40.25±41.34)μU/mL,and the difference was statistically significant(P<0.05).There was no significant difference in the area of the other glucose and insulin curves and the islet beta cell function index(P>0.05).Conclusion When the amount of food is relatively fixed,the breakfast time are prolonged,and the insulin secretion is longer after the meal.
