摘要
目的评价多层螺旋CT扫描在不典型肝脓肿诊断的价值。方法 2015年11月—2018年1月,医院共收治不典型肝脓肿56例(115个病灶)纳入观察组,另外选择同期诊断的肝癌消融后复发、肝转移瘤患者20例合计35个病灶纳入对照组,均进行了CT增强扫描。结果胆源性脓肿(n=60)、门静脉源性炎性水肿(n=50)、肝动脉源性与临近组织器官蔓延炎性水肿(n=5)表现为相应部位的CT扫描特征。对照组肿瘤病患者的囊腔及周围肿瘤组织面积倍数为(2.75±1.75)、簇状与融合表现占14.3%(5/35),低于观察组肝脓肿囊腔及周围水肿组织面积倍数(11.5±7.4)、82.6%(95/115),差异有统计学意义(P<0.05)。水肿肝实质平扫密度低于正常肝实质,延迟期密度高于正常肝实质,差异有统计学意义(P<0.05)。观察组环壁完整、环内低密度率高于对照组,观察组向心性延迟强化率低于对照组,差异有统计学意义(P<0.05)。结论多层螺旋CT扫描可定性诊断不典型肝脓肿,同时还能分析病因,指导疾病的治疗。
Objective To evaluate the diagnostic value of multi-slice spiral CT in atypical hepatic abscess.Methods From November 2015 to January 2018,56 cases of atypical liver abscess(115 lesions)were admitted to the observation group.In addition,20 patients with recurrence of liver cancer and 35 liver metastases were included in the control group at the same time.The CT enhanced scan was performed.Results Biliary abscess(n=60),portal venous inflammatory edema(n=50),hepatic arterial origin,and adjacent tissue and or gan spread inflammatory edema(n=5)were characterized by CT scanning in the corresponding site.The area of the cystic cavity and surrounding tumor tissue of the control group was(2.75±1.75),cluster and fusion was 14.3%(5/35),which was lower than that of the observation group(11.5±7.4)and 82.6%(95/115),and the difference was statistically significant(P<0.05).The density of liver edema was lower than that of normal liver parenchyma,and the density of delayed phase was higher than that of normal liver parenchyma.The difference was statistically significant(P<0.05).The ring wall of the observation group was complete and the rate of low density in the ring was higher than that of the control group,and the centripetal delay enhancement rate of the observation group was lower than that of the control group,the difference was statistically significant(P<0.05).Conclusion Multi-slice spiral CT scan can qualitatively diagnose atypical liver abscess,and can also analyze the etiology and guide the treatment of the disease.
