磁共振平扫及MRCP成像对十二指肠憩室诊断的临床价值分析研究 被引量:4

摘要 目的分析十二指肠憩室MRI和磁共振胆胰管成像(MRCP)的影像表现,探讨十二指肠乳头旁憩室(PAD)的临床价值及意义。方法选取2015年10月~2017年10月我院经上消化道造影、胆管造影或内镜检查证实的,并已行MRI、MRCP检查的46例十二指肠憩室患者,分析憩室的发生部位、大小和信号特征,以及胆管、胰腺及其他伴发征象相关性。结果 46例十二指肠憩室的患者共计发现憩室49个,43例为单发憩室,3例为多发憩室,直径以10~25 mm者居多,18个(40.82%)憩室内为纯液体未见气体成分;3个(6.12%)憩室内含气未见液性成分;23个(53.06%)憩室内气液混合、含气液平面;5个(10.20%)为食物残留等混杂成分。结论 MRI及MRCP对十二指肠憩室诊断具有较高的准确性,对临床治疗有重要的指导意义。 Objective To analyze the MRI and magnetic resonance cholangiopancreatography(MRCP)manifestations of duodenal diverticulum,and to expore the clinical value and significance of periampullary diverticulum(PAD).Methods 46 patients with duodenal diverticulum who underwent MRI and MRCP confirmed by upper gastrointestinal tract imaging,cholangiography or endoscopy from October 2015 to October 2017 were retrospectively analyzed.The diverticulum location,size and signal characteristics,as well as the correlations between bile duct,pancreas and other associated signs were analyzed.Results There were a total of 49 diverticula in 46 patients with duodenal diverticulum,43 of whom were single diverticulum,and 3 of whom were multiple diverticula.Most of them were with the diameter of 10-25 mm.18(40.82%)diverticula contained fluid alone,without gas composition;3(6.12%)diverticula contained gas alone,without fluid composition;23(53.06%)diverticula contained gas-liquid mixture and gas-liquid level;5(10.20%)diverticula contained mixed ingredients such as food residues.Conclusion MRI and MRCP have a high accuracy in the diagnosis of duodenal diverticulum,which has an important guiding significance for clinical treatment.
