摘要
目的:观察肠内营养支持联合甲氧氯普胺对机械通气患者的治疗效果。方法:回顾性分析ICU收治的机械通气早期营养患者193例,对照组90例在常规治疗基础上给予肠外营养支持,观察组103例患者给予肠内营养支持联合甲氧氯普胺,比较两组治疗前后营养指标、机械通气时间、1周内呼吸机相关性肺炎(VAP)发生率、不良反应发生率。结果:两组患者入院时的NR2002评分、血清前白蛋白、血红蛋白、总蛋白等指标比较差异无统计学意义(P>0.05);观察组患者治疗1周后NR2002评分、血清前白蛋白等指标明显优于对照组,且差异具有统计学意义(P<0.05);观察组治疗第3、7天的血糖水平明显低于对照组,比较差异有统计学意义(P<0.05);观察组患者返流、消化道出血等发生率明显低于对照组(P<0.05);观察组患者7 d营养费用、28 d治疗费用及住院天数与对照组比较差异不明显(P>0.05);观察组患者机械通气时间明显低于对照组(P<0.05)。结论:肠内营养支持联合甲氧氯普胺可以改善机械通气患者营养状态,利于缩短机械通气时间,且未明显增加不良反应。
Objective:To observe effects of Metoclopramide combined with enteral nutrition support in patients with mechanical ventilation.Methods:The clinical data of 193 patients with mechanical ventilation in ICU were retrospectively analyzed.They were divided into control group(n=90)and observation group(n=103).The control group was given routine therapy and parenteral nutrition support,while the observation group used Metoclopramide combined with enteral nutrition support.The nutritional indexes before and after the treatment,the mechanical ventilation time,the 1 week incidence of ventilator associated pneumonia(VAP),and the incidence of adverse reactions of the two groups were compared.Results:There were no statistical differences in the NR2002 score and the levels of serum albumin,hemoglobin and total protein between the two groups at admission(P>0.05);however,these indexes were better in the observation group than those in the control group 1 week after the treatment,and the differences were statistically significant(P<0.05).The blood glucose levels 3 and 7 days after the treatment of the observation group were significantly lower than those of the control group,and the differences were statistically significant(P<0.05).The incidence rates of reflux and gastrointestinal bleeding of the observation group were significantly lower than those of the control group(P<0.05),and the mechanical ventilation time was significantly lower than that of the control group(P<0.05).There were no significant differences in the 7d nutrition costs,28d treatment costs and hospitalization time between the two groups(P>0.05).Conclusions:Metoclopramide combined with enteral nutrition support in the patients with mechanical ventilation can improve the patients'nutrition state and shorten the mechanical ventilation time,but cannot increase the adverse reactions obviously.
