摘要
目的:探讨儿童体质与血清肌酸激酶同工酶(CK-MB)水平的相关性,并评估其临床价值.方法:选择116例1~3岁肺炎心衰患儿(男60例,女56例,心功能Ⅱ级)作为观察组,并选择正常体检的98名1~3岁儿童(男50例,女48例)作为对照组.测量观察对象的身高、体重,根据幼儿Kaup指数将患儿分为三组:正常组(Kaup为13.5~18.0)、优良组(Kaup为18.1~20.0)和肥胖组(Kaup>20.1).测定受试者血清CK-MB、超敏C反应蛋白(hsCRP)水平,并对检测结果进行分析.结果:观察组各组CK-MB水平均高于对照组(均P=0.000),CK-MB水平随Kaup水平增加而逐渐升高,CK-MB与Kaup水平之间存在显著正相关(r=0.824,P=0.000),CK-MB与hsCRP水平两者之间也存在显著正相关(r=0.660,P=0.000).结论:体质状况显著影响心衰患儿血清CK-MB水平,血清CK-MB水平随肥胖程度增加而升高,单纯依赖CK-MB水平检查不利于肥胖患儿心衰的诊断和预后判断.
Objective To explore the effect of physical constitution on serum levels of Creatine kinase Isoenzyme(CK-MB)in children,and to assess its clinical values.Methods 116 patients aged 1-3 years with pneumonia complicated by heart failure were enrolled in this study(60 males,65 females,and cardiac function gradeⅡ)as observation group,98 healthy children aged 1-3 years(50 males,48 females)were enrolled as control group.The height and weight of each object were measured,and infant Kaup index were calculated with these parameter.The objects were divided into 3 groups according to Kaup index:normal group(Kaup13.5-18.0),excellent group(Kaup 18.1-20.0)and obese group(Kaup>20.0).The serum levels of CK-MB and hypersensitive C-reactive protein(hsCRP)were detected in the objects,respectively,and the result were analyzed.Results The levels of CK-MB were significantly higher in observation groups compared to control groups respectively(all P=0.000),they increased with the rise of Kaup,and were apparent significantly positive correlated with Kaup(r=0.824,P=0.000).The levels of CK-MB were apparent significantly positive correlated with the levels of hsCRP too(r=0.660,P=0.000).Conclusions In children with heart failure,plasma CK-MB levels are affacted significantly by physical constitution,the levels of CK-MB increased with the rise of Kaup,so relying only on levels of CK-MB is not conducive to the diagnosis and prognosis for obesity patients with heart failure.
