摘要
目的探讨宫颈管诊刮(ECC)对宫颈病变的诊断价值。方法分析2017年1—10月因宫颈癌筛查结果异常行阴道镜检查及宫颈活检同时行宫颈管诊刮的992例患者的组织病理学结果。结果 HSIL、ASC-H两组阳性率均高于LSIL、ASC-US、NILM三组,差异有统计学意义(P<0.01)。LSIL、ASC-US、NILM组阳性率较低,三组间差异无统计学意义(P>0.05)。ECC比宫颈活检检出更高级别病变53例中,3型转化区占86.8%(46/53)。结论 ECC能够提高检出宫颈病变的阳性率,建议对宫颈细胞学HSIL、ASC-H,或阴道镜下3型转化区时宫颈活检联合ECC以增加宫颈病变的检出,减少漏诊。
Objective To explore the diagnostic value of ECC for cervical lesions.Methods The histopathological results of 992 patients with abnormal vaginal examination and cervical biopsy at the same time with cervical cancer screening results from January to October 2017 were analyzed.Results The positive rate of HSIL,ASC-H group was significantly higher than that of LSIL,ASC-US and NILM group,and the difference was statistically significant(P<0.01).The positive rate of LSIL,ASC-US and NILM group was lower,and there was no statistically significant difference among the three groups(P>0.05).Of the 53 cases with higher levels of ECC than cervical biopsies,the 3-type conversion area accounted for 86.8%(46/53).Conclusion ECC can improve the positive rate of detection of cervical lesions,for HSIL cervical cytology,ASC-H,or type 3 transformation zone,cervical biopsy under colposcope joint ECC can increase the detection of cervical lesions,reduce misdiagnosis.
