摘要
目的观察临床应用高频振荡通气(HFOV)联合肺表面活性物质(PS)治疗新生儿肺出血(NPH)的效果。方法选取我院治疗的90例肺出血新生儿,随机分为对照组和观察组各45例,对照组患儿应用高频振荡通气治疗,观察组则联合给予肺表面活性物质治疗,对比分析两组患者治疗后的临床疗效。结果给予高频振荡通气治疗前两组患儿Pa O_2、Pa CO_2水平比较差异无统计学意义(P>0.05),通气后6h两组患儿Pa O_2、Pa CO_2水平均较通气前明显改善(P<0.05),通气后24h则较通气后6h明显改善(P<0.05),对照组患儿通气后各时间段Pa O_2、Pa CO_2水平改善幅度明显低于观察组(P<0.05),两组患儿通气时间、肺出血停止时间以及住院时间比较观察组均明显少于对照组(P<0.05),观察组患儿不良反应发生率明显低于对照组(P<0.05),观察组患儿治愈率明显高于对照组(P<0.05)。结论临床应用高频振荡通气联合肺表面活性物质治疗新生儿肺出血可有效缩短治疗时间,改善患儿血气指标,提高临床治愈率。
Objective To observe the clinical application of high frequency oscillatory ventilation(HFOV)combined with lung surface active substances(PS)in treatment of neonatal pulmonary hemorrhage(NPH).Methods From April 2015 to October 2016 in our early treatment,90 cases of neonatal pulmonary bleeding were randomly divided into the control group and the observation group,45 cases in each group.The control group was given high frequency oscillatory ventilation therapy,and the observation group was combined with lung surface active substance treatment.The clinical curative effect was compared between the two groups.Results Before treatment,the levels of PaO 2 and PaCO 2 had no statistical significance between the two groups(P>0.05).6h after ventilation,the levels of PaO 2 and PaCO 2 improved significantly(P<0.05),and it was even better 24 h after ventilation(P<0.05).The improvement was more pronounced in the observation group than in the control group at each period(P<0.05).Their ventilation time,pulmonary hemorrhage,and hospital stay were significantly shorter in the observation group than in the control group(P<0.05),and the incidence of adverse reactions was obviously lower in observation group than in the control group(P<0.05).The cure rate was obviously higher in the observation group than in the control group(P<0.05).Conclusion The clinical application of high frequency oscillatory ventilation combined with lung surface active substance can effectively shorten the treatment time,improve blood gas index and clinical cure rate in treatment of neonatal pulmonary hemorrhage.
