摘要
目的探讨对静脉留置针及输液器管路自制负压系统穿刺与传统脓肿引流术治疗哺乳期乳腺炎乳腺脓肿的临床效果。方法将我科2015年12月至2016年12月收治的65例哺乳期乳腺炎乳腺脓肿患者随机分为观察组(n=36)和对照组(n=29),分别行静脉留置针及输液器管路自制负压系统穿刺和传统脓肿引流术。观察两组患者的愈合时间、换药频次、疼痛缓解程度、乳房变形情况及回乳情况。结果观察组愈合时间、换药频率明显优于对照组(P<0.05)。手术后,两组患者的VAS评分均明显低于术前,且观察组VAS评分明显低于对照组(P<0.05)。观察组患者无乳房变形情况出现,对照组患者5例(17.24%)存在乳房变形,观察组乳房变形发生率明显低于对照组(P<0.05)。观察组回乳率(2.78%)明显低于对照组(48.28%),差异具有统计学意义(P<0.05)。结论与传统脓肿引流术比较,自制负压管路系统治疗乳腺脓肿疗效更显著,可加快脓腔愈合,减轻患者痛苦且明显降低回乳率,美容效果佳,安全可靠。
Objective To analyze the clinical efficacy of self-made negative pressure systemand system with venous indwelling needle and transfusion device and conventional abscess drainage in the treatment of breast abscess caused by lactation mastitis.Methods From December 2015 to December 2016,65 patients with breast abscess caused by lactation mastitis were randomly divided into observation group(n=36)and control group(n=29).The control group was given conventional abscess drainage,and the observation group was treated with self-made negative pressure systemand system with venous indwelling needle and transfusion device.The healing time,frequency of dressing change,pain relief,breast deformation and breast regurgitation were compared between the two groups.Results The healing time and frequency of dressing change of the observation group were better than those of the control group(P<0.05).After operation,the VAS scores of the two groups were lower than those before operation,and that of the observation group was lower than the control group(P<0.05).There was no breast deformation in the observation group,and 5 cases(17.24%)of the control group had breast deformation,and the incidence of breast deformation in the observation group was significantly lower than that in the control group(P<0.05).The rate of breast regurgitation in the observation group was 2.78%,which was lower than 48.28%of the control group,the difference was statistically significant(P<0.05).Conclusion Compared with the conventional abscess drainage,self-made negative pressure systemand system with venous indwelling needle and transfusion device in the treatment of breast abscess caused by lactation mastitis has more remarkable clinical effect.It can shorten the abscessus healing time,reduce pain and breast regurgitation rate and has good beauty effect,which is safe and reliable.
