摘要
目的分析2型糖尿病(T2DM)患者蛋白尿程度与振动感觉阈值(VPT)的关系。方法选取2015年7月—2017年7月该院收治的380例T2DM患者,按照患者蛋白尿程度的不同将其分为无蛋白尿(NAU)组(152例)、微量蛋白尿(MAU)组(143例)和大量蛋白尿(OAU)组(85例),比较3组患者的VPT,分析蛋白尿程度与VPT的关系。结果 MAU组和OAU组患者糖尿病病程分别为(8.3±3.3)年、(9.4±4.3)年,明显长于NAU组的(6.2±1.6)年;MAU组UACR、HbA1c、VPT水平分别为(90.0±63.7)g/mg、(8.7±2.8)%、(18.4±9.8)V,OAU组UACR、Hb A1c、VPT水平分别为(950.2±300.6)g/mg、(9.0±2.1)%、(26.7±10.6)V,两组UACR、HbA1c、VPT均明显高于NAU组,组间差异有统计学意义(P<0.05)。UACR与年龄、糖尿病病程、BMI、HbA1c、FPG、2 hPG、BUN、Scr、VPT成正相关关系,糖尿病病程与HbA1c是UACR的独立危险因素(P<0.05)。结论 T2DM患者蛋白尿程度与VPT呈正相关关系,随着患者蛋白尿程度的加重,VPT异常率会随之提高。
Objective This paper tries to analyze the relationship of the degree of proteinuria and vibration perception threshold(VPT)in type 2 diabetes mellitus(T2DM)patients.Methods 380 patients with T2DM treated in this hospital from July 2015 to July 2017 were selected,according to the degree of proteinuria,they were divided into non-proteinuria group(152 cases),microalbuminuria group(143 cases)and mass-proteinuria group(85 cases),the VPT of the three groups were compared,and the relationship between proteinuria and VPT was analyzed.Results The courses of diabetes in the MAU group and OAU group were(8.3±3.3)years and(9.4±4.3)years,significantly longer than the NAU group of(6.2±1.6)years;The levels of UACR,HbA1c and VPT in the MAU group were(90.0±63.7)g/mg,(8.7±2.8)%,(18.4±9.8)V,the levels of UACR,HbA1c and VPT in the OAU group were(950.2±300.6)g/mg,(9.0±2.1)%,(26.7±10.6)V.The UACR,HbA1c,VPT in the two groups were significantly higher than those of the NAU group(P<0.05),with statistically significant differences.UACR had positive correlation with age,duration of diabetes,BMI,HbA1c,FPG,2 hPG,BUN,Scr,VPT,the duration of diabetes and HbA1c were the independent risk factors of UACR(P<0.05).Conclusion The degree of albuminuria in T2DM patients is positively related to VPT,and the abnormal rate of VPT increases with the aggravation of proteinuria.
