Clinical Effect of Early Continuous Enteral Nutrition Combined with Huchang Mixture in Treating Severe Pancreatitis and Its Effect on Immune Function

摘要 目的观察早期持续肠内营养联合护肠合剂治疗重症胰腺炎的临床疗效及对免疫功能的影响。方法选择医院2014年1月至2017年1月收治的重症胰腺炎患者100例,按随机数字表法分为对照组和观察组,各50例。对照组患者应用早期持续肠内营养支持治疗,观察组患者在此基础上联用护肠合剂。观察周期为10 d。结果经营养支持治疗后,观察组患者腹胀痛缓解时间,体温恢复时间,血、尿淀粉酶恢复时间均明显短于对照组,白蛋白(ALB)及前白蛋白(PA)水平均明显高于对照组(P<0.01);治疗后,观察组患者免疫功能指标(CD3+,CD4+,CD8+及CD4+/CD8+)明显高于对照组(P<0.05),C反应蛋白(CRP)、白细胞计数、白细胞介素6(IL-6)和肿瘤坏死因子-α(TNF-α)水平均低于对照组(P<0.01);两组患者不良反应发生率相当(P>0.05)。结论早期持续肠内营养联合护肠合剂治疗重症胰腺炎可减轻症状及体征,消除炎性反应,改善免疫功能,值得临床推广。 Objective To observe the clinical effect of early continuous enteral nutrition combined with Huchang Mixture in the treatment of patients with severe acute pancreatitis(SAP)and its effect on immune function.Methods Totally 100 patients with SAP admitted to our hospital from January 2014 to January 2017 were selected and divided into the control group and the observation group according to the random number table method,50 cases in each group.The control group was given early continuous enteral nutrition,on this basis,the observation group was given Huchang Mixture.The observation period was 10 d.Results After the treatment of nutritional support,the relief time of abdominal distension and pain,the recovery time of body temperature and the recovery time of blood and urine amylase in the observation group were significantly shorter than those in the control group,and the levels of ALB and PA in the observation group were higher than those in the control group(P<0.01).After treatment,the immune function indexes(CD3+,CD4+,CD8+and CD4+/CD8+)in the observation group were significantly higher than those in the control group(P<0.05),the levels of CRP,WBC,IL-6 and TNF-αin the observation group were lower than those in the control group(P<0.01).There was no significant difference in the incidence rate of adverse reactions between the two groups(P>0.05).Conclusion Early continuous nutrition enteral nutrition combined with Huchang Mixture in the treatment of patients with SAP can relieve symptoms and signs,eliminate inflammatory reactions and improve immune function,which is worthy of clinical promotion.
