摘要
目的:探讨灯盏花素辅助治疗对重症急性胰腺炎患者炎症因子、免疫功能和血液流变学的影响。方法:将86例重症急性胰腺炎患者随机分为观察组和对照组,各43例,观察组在奥曲肽局部动脉灌注治疗的基础上,加用灯盏花素持续性动脉灌注进行辅助治疗,对照组仅应用奥曲肽局部动脉灌注进行治疗。比较治疗前、后两组患者炎症因子、免疫功能和血液流变学变化。结果:与治疗前相比,治疗后两组患者炎症因子C反应蛋白(hs-CRP)、白介素-6(IL-6)、肿瘤坏死因子α(TNF-α)水平均显著降低(P<0.05),观察组下降更显著(P<0.05);与治疗前相比,治疗后两组患者免疫功能指标CD4细胞比例及CD4/CD8比值均显著升高(P<0.05),观察组升高更显著(P<0.05),CD8细胞比例差异无统计学意义(P>0.05);与治疗前相比,治疗后两组患者血液流变学指标高切全血比黏度、血浆比黏度、血小板黏附率、血栓长度均显著降低(P<0.05),观察组下降更显著(P<0.05)。结论:灯盏花素辅助治疗对重症急性胰腺炎患者疗效显著,可明显改善炎症因子、免疫功能和血液流变学水平。
Objective：To investigate the effect of adjuvant therapy with breviscapine on inflammatory factors,immune function and hemorheology in patients with severe acute pancreatitis.Methods：A total of 86 severe acute pancreatitis patients were divided into observation group（43cases）and control group（43cases）.The two groups were given the arterial perfusion of octretide and the observation group was additionally given continuously arterial perfusion of breviscapine.Levels of inflammatory factors,immune function and hemorheology were tested and compared.Results：The two groups inflammatory factors（hs-CRP,IL-6,TNF-α）levels were decreased significantly after treatment,and the observation group decreased more than that of the control group with significantly（P〈0.05）;The two groups the percenrage of CD4 and CD4/CD8 were increased significantly after treatment,and the observation group increased more than that of the control group with significantly（P〈0.05）,there was no significant difference in the percenrage of CD8;The two groups hemorheology（blood high shear viscosity,plasma viscosity,platelet adhesion rate,thrombus length）levels were decreased significantly after treatment,and the observation group decreased more than that of the control group（P〈0.05）.Conclusion：Adjuvant therapy with breviscapine can obviously reduce inflammation,and improve immune function and hemorheology.
