摘要
目的研究血清基质金属蛋白酶9(MMP-9)、组织多肽特异性抗原(TPS)水平变化与肺癌患者TNM分期的关联性分析。方法选取2015年3月—2017年10月本院肺癌患者112例,均应用酶联免疫吸附法检测血清MMP-9、TPS表达水平,并对比不同TNM分期肺癌患者血清MMP-9、TPS表达水平,分析血清MMP-9、TPS表达水平与TNM分期关联性。结果肺癌患者血清MMP-9、TPS阳性率随TNM分期升高而逐渐升高,且不同分期间阳性率对比,差异有统计学意义(P<0.05);经单因素方差分析,不同TNM分期间血清MMP-9、TPS表达水平对比,差异有统计学意义(P<0.05);两两比较,Ⅳ期高于Ⅲ期,Ⅲ期高于Ⅱ期,Ⅱ期高于Ⅰ期,差异有统计学意义(P<0.05);采用ROC曲线分析,血清TPS、MMP-9表达水平判断早期、晚期肺癌患者,差异有统计学意义(P<0.05),其分界值为135.60μg/L、374.01ng/mL;经Spearman分析,血清MMP-9、TPS表达水平与肺癌患者TNM分期呈正相关(r1=0.586,P1=0.009;r2=0.871,P2=0.006)。结论血清MMP-9、TPS表达水平与肺癌患者TNM分期密切相关,可应用于临床评估患者病情进展程度,为治疗及预后判断提供依据。
Objective To study the correlation analysis of serum matrix metalloproteinase 9(MMP-9),tissue polypeptide specific antigen(TPS)level and TNM staging in patients with lung cancer.Methods 112 cases of lung cancer patients were selected in our hospital from Mar.2015 to Oct.2017.The levels of serum MMP-9 and TPS were detected by enzyme linked immunosorbent assay(ELISA).The expression of MMP-9 and TPS in the serum of patients with lung cancer in different TNM stages was compared and the correlation between the expression level of serum MMP-9 and TPS and the stage of TNM staging was analyzed.Results The positive rate of MMP-9 and TPS in the serum of lung cancer patients increased gradually with the increase of TNM staging.The difference of positive rate in different stages was statistically significant(P<0.05);By single factor analysis of variance,the levels of MMP-9 and TPS in different TNM stages were statistically significant(P<0.05);Having a pairwise comparison,stage IV was higher than stage III,stage III was higher than stage II,stage II was higher than stage I,and the difference was statistically significant(P<0.05);By ROC curve analysis,the expression level of serum TPS and MMP-9 in early and late stage lung cancer patients was statistically significant(P<0.05),and the cut-off value was 135.60 g/L,374.01 ng/mL;After Spearman analysis,the expression of serum MMP-9 and TPS was positively correlated with the TNM staging of patients with lung cancer(r 1=0.586,P 1=0.009;r 2=0.871,P 2=0.006).Conclusion The expression level of serum MMP-9 and TPS is closely related to TNM staging in patients with lung cancer.It can be applied to evaluate the degree of disease progression and provide basis for treatment and prognosis.
