摘要
目的对比重型颅脑损伤患者在气管切开后使用不同吸痰方法给其早期肺通气功能带来的影响,并观察患者的肺部感染状况。方法方便择取2014年12月—2016年12月期间于该院就诊的80例重型颅脑损伤患者,全部行气管切开操作,并为其使用不同吸痰方法。按照患者使用的不同吸痰方法将其分为两组,分别为采用常规吸痰法的40例对照组患者、采用改良吸痰法的40例研究组患者。对比两组患者的肺部感染状况及早期肺通气功能指标。结果研究组肺部感染发生率仅5.00%(2/40),对照组肺部感染发生率则为20.00%(8/40);研究组气道黏膜出血发生率为7.50%(3/40),对照组气道黏膜出血发生率则为30.00%(12/40);此外,研究组患者的氧分压与氧合指数均高于对照组,其二氧化碳分压低于对照组,差异有统计学意义(P<0.05)。结论相较于常规吸痰法,为重型颅脑损伤患者在气管切开后使用改良吸痰法,可以更加有效预防肺部感染、改善早期肺通气功能。
Objective This paper tries to observe the effect of different sputum m ethods on early pulm onary ventilation function after tracheotom y in patients with severe craniocerebral injury and to observe the pulm onary infection in patients.Methods 80 patients with severe craniocerebral injury from D ecem ber 2014 to D ecem ber 2016 were treated with tracheotomy and different aspiration m ethods.The patients were convenient divided into two groups according to the different m ethods of aspiration.40 patients in the control group were treated with conventional aspiration m ethod and 40 patients with modified suction m ethod.The incidence of pulm onary infection and early pulm onary ventilation function were com pared between the two groups.Results The incidence of pulm onary infection in the study group was only 5.00%(2/40)and 20.00%(8/40)in the control group.The incidence of airway m ucosal bleeding was 7.50%(3/40),the incidence of airway m ucosal hem orrhage in the control group was 30.00%(12/40).In addition,the oxygen partial pressure and oxygen saturation of the study group were higher than the control group,the partial pressure of carbon dioxide was lower than that of the control group,the difference was statistically significant(P<0.05).Conclusion Compared with conventional sputum m ethod,the patients with severe craniocerebral injury can improve the pulm onary function and improve the early pulm onary ventilation function by using the modified suction m ethod after tracheotomy.
