摘要
目的探讨农村慢性肺源性心脏病患者实施长期家庭氧疗的应用价值。方法选取COPD合并慢性肺源性心脏病的农村患者86例,分为氧疗组48例和对照组38例。对照组给予常规治疗,氧疗组在此基础上辅以长期家庭氧疗。随访2年,比较治疗前及治疗1、2年后2组患者动脉血气分析、外周血象、超声心动图指标,比较2组患者复发情况及病死率。结果治疗1年后,氧疗组患者动脉p(O2)、外周血Hb水平及m PAP较治疗前和对照组显著改善(P<0.05);治疗2年后,氧疗组患者动脉p(CO2)、外周血RBC水平、右室流出道内径以及右室内径较治疗前和对照组显著改善(P<0.05)。氧疗组与对照组慢性肺源性心脏病患者人均复发次数比较,差异有统计学意义(P<0.05);氧疗组与对照组患者病死率差异无统计学意义(P>0.05)。结论农村肺心病患者实施家庭氧疗可有效提高动脉p(O2),降低p(CO2)及肺动脉压,改善贫血及右心功能,减少慢性肺源性心脏病复发。
Objective To explore the clinical value of long-term home oxygen therapy in treating patients with chronic pulmonary heart disease in rural areas.Methods Totally 86 COPD rural patients with chronic pulmonary heart disease were selected and divided into oxygen therapy group(n=48)and control group(n=38).The control group was given conventional therapy,while oxygen therapy group was given long-term home oxygen therapy on the basis of the control group.The patients were followed up for 2 years.The arterial blood gas analysis,peripheral blood and echocardiographic indicators of the two groups were compared before treatment and 1 year and 2 years after treatment.The recurrence and mortality of the two groups were compared.Results After 1 year of treatment,p(O2),the level of peripheral blood Hb and mPAP in the oxygen therapy group were significantly better than those before treatment and control group(P<0.05).Two years after treatment,p(CO2),peripheral blood level of RBC,inner diameter of right ventricular outflow tract P tract and right ventricular diameter in the oxygen therapy group were significantly better than those before treatment and the control group(P<0.05).There was significant difference in the recurrence frequency between the oxygen therapy group and the control group(P<0.05).There was no significant difference in mortality between the oxygen therapy group and the control group(P>0.05).Conclusion The implementation of home oxygen therapy for rural patients with chronic pulmonary heart disease in rural areas can effectively improve the arterial p(O2),reduce p(CO2)and pulmonary arterial pressure,improve anemia and right heart function,and reduce the recurrence of chronic pulmonary heart disease.
