18F-FDG PET/CT融合显像对首发骨骼病变诊断的增益价值 被引量:3

摘要 目的探讨18F-FDG PET/CT显像对首发骨骼病变准确诊断的增益价值。方法回顾性分析2013年3月至2015年3月患者因骨骼病变为首发症状在聊城市人民医院核医学科行PET/CT扫描的患者41例,记录PET部分的SUVmax,CT部分骨破坏特点,确诊依靠病理结果,数据分析采用独立样本t检验及χ~2检验。结果 PET/CT明确诊断多发骨转移瘤13例并找到原发灶,骨病灶的SUVmax(9.95±3.81);明确诊断骨髓瘤16例,11例溶骨破坏伴软组织形成的患者的SUVmax(8.75±3.96)与多发骨转移瘤差别无统计学意义(t=0.47,P>0.05),5例仅为溶骨性破坏患者的SUVmax(4.85±2.11)与多发骨转移瘤差别有统计学意义(t=43.34,P<0.05)。较罕见的恶性骨肿瘤SUVmax(12.9±1.72),其与骨结核的SUVmax13.4差别不大(t=1.86,P>0.05),但与其他良性骨肿瘤SUVmax(6.32±2.19)差别有统计学意义(t=2.95,P<0.05)。PET/CT扫描诊断准确率为90.2%(37/41),常规显像诊断准诊率为34.1%(14/41)两者差别有统计学意义(χ~2=12.25,P<0.05)。结论 18F-FDG PET/CT作为一种全身的解剖显像与功能显像同机融合的影像技术实现了1+1+1>3,对首发骨骼病变的准确诊断有较高的增益价值。 Objective:To evaluate the added value of 18 F-FDG PET/CT fusion imaging in the diagnosis on the initial bony lesions.Methods:41 patients underwent PET/CT fusion imaging were retrospectively reviewed and analysed in our department from March 2013 to March 2015.The final diagnoses were confirmed by follow-up pathological results.Statistical analysis was performed with independent t-test and Chi-square test.Results:13 multiple bony metastases were all detected primary tumor,the SUVmax on bony lesions was(9.95±3.81),16 multiple myeloma were definitely diagnosed,the SUVmax of 11/16 multiple bony destruction with soft tissue was 8.75±3.96(t=0.47,P>0.05)and 5/11 osteolytic bone lesions was 4.85±2.11(t=43.34,P<0.05).The SUVmax of rarely primary malignant bone tumors was(12.9±1.72),the SUVmax of a bone tuberculosis was 13.4(t=1.86,P>0.05),the SUVmax of other benign bone tumors was 6.32±2.19(t=2.95,P<0.05).The accuracy rate of diagnosis of PET/CT fusion imaging was remarkably higher than conventional imaging(χ2=12.25,P<0.05).Conclusion:As a whole-body anatomical imaging and functional imaging technology of the same machine,the 18 F-FDG PET/CT fusion imaging has achieved1+1+1>3,which has a higher added value for the accurate diagnosis of the initial bone lesions.
