摘要
目的探讨近期负性生活事件和心理痛苦对护理本科生肠易激综合征(irritable bowel syndrome,IBS)的影响,并分析心理痛苦在负性生活事件与IBS症状间的中介作用。方法 2017年1-7月,目的抽样法选取温州医科大学护理学院护理本科生为研究对象。采用病例-对照研究的方法,在调查的904名护理本科生中筛选出62名IBS患者作为病名组,同期以1∶2的配比获得124例健康对照。采用一般资料问卷、消化道症状严重程度问卷、青少年生活事件量表、抑郁自评量表和焦虑自评量表对其进行问卷调查。结果观察组护生的近期负性生活事件、焦虑、抑郁得分均高于对照组,差异有统计学意义(P<0.05)。Logistic回归分析显示,抑郁及近期负性生活事件中的人际关系是护生IBS发病的危险因素。分层回归分析显示,心理痛苦在近期负性生活事件与IBS症状间起完全中介作用。结论促进IBS护生心理健康以增强其应对负性生活事件的能力,将有助于其IBS症状的自我管理。
Objective To explore the effect of recent negative life events and psychological distress on the irritable bowel syndrome(IBS),and to analyze the mediating effect of psychological distress between negative life events and IBS symptoms.Methods By using purposive sampling method,the nursing undergraduates of Wenzhou medical university school of nursing were selected.A case-control study was conducted.Among 904 undergraduate nursing students who completed the investigation,62 students were selected as eligible IBS patients,and another 124 students were selected as health controls in a 1:2 ratio.Demographic questionnaire,severity of digestive symptom questionnaire,adolescent self-rating life events checklist,self-rating depression scale,and self-rating anxiety scale were employed for investigation.Results The scores of recent negative life events,anxiety and depression were higher in observation group than these of control group(P<0.05).Logistic regression revealed that the interpersonal relationship in adolescent self-rating life events checklist and depression were the risk factors of IBS.The hierarchic regression indicated psychological distress played full mediation role between recent negative life events and IBS symptom.Conclusion Promotion of nursing students’psychological health in order to improve their ability to deal with negative life events will be helpful for the symptom self-management in IBS nursing students.
