摘要
目的探究会阴侧切对生产困难征象初产妇产后盆底功能和血清炎症因子的影响。方法选取自2017年1月1日~6月1日间我院接收的具有生产困难征象的初产妇84例作为研究对象,按产妇意愿分为:A(自然分娩)、B(常规会阴侧切)两组,观察两组产妇产后盆底肌功能异常率和产前产后相关炎症因子的变化。结果 B组产妇产后盆底肌功能各项指标异常发生率显著低于A组,差异有统计学意义(P<0.05);两组初产妇生产后血清IL-1、IL-6、IL-8、IL-10和TNF-α表达量均显著升高,差异有统计学意义(P<0.05);B组初产妇产后血清IL-1、IL-6、IL-8和TNF-α的表达量明显低于A组,差异有统计学意义(P<0.05)。结论会阴侧切术能够在一定程度上减少对初产妇的会阴盆底功能的损伤,同时降低相关促炎因子的表达,值得针对生产不顺的产妇采用。
Objective To investigate the effect of episiotomy on pelvic floor function and serum inflammatory factors in primiparas with birth difficulties.Methods From January1to June1,2017,84cases of primiparas with difficulties in childbirth were selected as the research subjects.They were divided into two groups according to their wishes:A(natural childbirth)and B(conventional episiotomy).The abnormal rate of pelvic floor muscle function and the changes of inflammatory factors related to prenatal and postpartum after delivery were observed in two groups of parturients.Results The abnormal rate of pelvic floor muscle function in B group was significantly lower than that in A group,and the difference was statistically significant(P<0.05).The expression of IL-1,IL-6,IL-8,IL-10and TNF-alpha in the serum of two groups of primiparas increased significantly,the difference was statistically significant(P<0.05).The expression of IL-1,IL-6,IL-8and TNF-alpha in serum of B group was significantly lower than that in group A,and the difference was statistically significant(P<0.05).Conclusion Lateral episiotomy can reduce the perineal pelvic floor function of primiparas to a certain extent,and reduce the expression of related proinflammatory factors.It is worthy of applying to the parturients with poor delivery.
