摘要
目前,痹证的发病率日渐攀升,尚没有有效的治疗方案可以完全治愈风湿免疫类疾病,对其症状的缓解和治疗,提高患者的生活质量则显得尤为重要,近年来大量动物实验和临床研究证明,脾虚证与免疫系统的异常有着密切的关系,目前中医治疗痹证,治本多以补益肝肾、养阴补血为主。而风湿免疫类疾病的患者,多有长期服药的历史,久药易伤脾胃,脾胃功能不全则药物的吸收,水谷精微的吸收都受到影响。脾胃健运,则气血生化有源,气充血足,脉道通利,四肢有所温养,则人体不易感受外邪;反之若脾胃虚弱,气血化源不足,气血亏虚,则易感受风寒湿邪,发为痹证。现代药理学和多种动物实验亦显示,脾的功能的强弱与痹证的发生与否有着统计学上的关联。因此治疗此类疾病当固其后天之本,方能药到病除。宋林萱教授在治疗弊证的过程当中,思路开阔,除了使用传统中医学辨证论治的思想,在治疗各种结缔组织病的过程当中,时时注重调理脾胃,并采用补中益气汤治疗各类风湿免疫疾病,均取得了很好的疗效。本文介绍宋林萱教授运用补中益气汤治疗类风湿关节炎、皮肌炎、骨关节炎、白塞病和干燥综合征的典型病例,治疗过程中辨证准确,组方合理,疗效确切,是宋林萱教授从脾论治痹证的经验小结。
At present,the incidence of the Bi syndrome(痹证)is increasing,there is no effective treatment for rheumatism and immune diseases,it is very important to relieve and treat the symptoms and improve the quality of life of the patients.In recent years,a large number of animal experiments and clinical studies have proved that the Pixu syndrome(脾虚证)is closely related to the abnormal immune system.At present,Buyi Ganshen(补益肝肾)and Yangyin Buxue(养阴补血)was the main principles in treating the Bi syndrome in TCM.And many rheumatism immune disease patients has long-term medication history,the spleen and stomach was easy to hurt,and the insufficiency of the spleen and stomach,the absorption of drugs and the absorption of water,grains and essence are all affected.The normal function of the spleen and stomach guarantees the health of Qi(气),blood,arteries and veins,and limbs.On the other hand,if the spleen and stomach is weak,the source of Qi and blood is insufficient,which makes it easy for the wind,cold and dampness evil to invade,which manifests as arthromyodynia.Modern pharmacology and a variety of animal experiments also show that the function of the spleen and the occurrence of paralysis are statistically related.Therefore,the treatment of such diseases should pay attention to the strengthening of the spleen.Professor Song Linxuan is open-minded in the process of treatment of illness.In addition to applying TCM dialectical treatment in treating various connective tissue diseases,attention is given to regulate the spleen and stomach.And the Buzhong Yiqi decoction(补中益气汤)is used to treat various rheumatic immune diseases,which has achieved good curative effects.This article introduces professor Song Linxuan's typical cases of treating rheumatoid arthritis,dermatomyositis,osteoarthritis,Behcet's disease and Sjogren's syndrome with the Buzhong Yiqi decoction in addition to summarizing Professor Song Linxuan's experience in treating arthralgia from spleen.
