摘要
通过对一年级住院医师进行临床胜任力自评,以评估不同类型住院医师在培训开始时的临床胜任力是否存在显著差异。选取2016年武汉大学人民医院全部一年级住院医师为调查对象,采用横断面问卷调查方法,本研究使用的临床胜任力结构包含:职业精神、患者管理、沟通和团队协作、学术和医疗保障体制5个维度,共15个项目,按美国住院医师的临床胜任力标准进行能力分层。根据自评水平是否达到相应标准计算整体维度和项目达标率。有效问卷302份,涉及23个培训专业。不同身份和学位类型住院医师的临床胜任力达标率比较中,单位人112.1/116(96.67%)、委培单位人15.7/16(98.33%)高于社会人53.4/57(93.68%)和4证合一104.1/113(92.09%),博士组46.3/47(98.44%)高于学士组172.9/186(92.94%)。因此,在制定住院医师的培训时间和内容标准时,需要综合考虑住院医师的身份和学位差异,进行临床胜任力评估,以更有针对性地培训。
This research was aimed to evaluate whether there is a significant difference in clinical competence among different types of residents when programs began,through self-assessment from first-year residents.The study conducted a cross-sectional survey of all the first-year residents in Renmin Hospital of Wuhan University in2016.The survey of clinical competence assessment included5domains(professionalism,patient care,communication and teamwork,academic performance,and quality control systems)with15items,and3levels were designed for each item based on ACGME clinical competence levels.The accomplished rates were counted from self-assessment results.There were302validated questionnaires receiving from23specialties.The comparison of competence accomplishment among different types of residents indicated that residents employed by Renmin Hospital of Wuhan University112.1/116(96.67%)and residents from other hospitals15.7/16(98.33%)had better results than social residents53.4/57(93.68%)and residents of graduates104.1/113(92.09%),and residents with doctor degrees46.3/47(98.44%)had better results than residents with bachelor degrees172.9/186(92.94%).It came to a conclusion that the types of residents need to be considered during making training plan and standards,and it’s necessary to evaluate the competency so that residents can get more personalized training.
